Job description

Job Description:
This position is part of our MindHealthy division. At MindHealthy®, we believe mental health is health.
Everyone deserves the chance to thrive—which is why we’re transforming behavioral health care with tech-enabled, compassionate, whole-person support.
We’re hiring Clinical Behavioral Care Managers to join our team using the Collaborative Care Model (CoCM)—a proven, team-based approach that integrates mental health into primary care. It’s designed to make care more accessible, coordinated, and effective, especially for conditions like depression and anxiety.
Why CoCM?
  • Better Outcomes: Patients improve faster with regular check-ins, care coordination, and data-driven tracking.
  • Less Stigma: Mental health care happens in primary care—no labels, no separate clinics.
  • More Access: Reach more people, especially where specialists are scarce.

What Makes It Different?
CoCM focuses on short-term, measurable progress for mild to moderate conditions. You’ll use tools like the PHQ-9 to track symptoms and adjust care as needed. The goal? Help patients “graduate” from care.
This role is a good fit for social workers who are comfortable with structure, thrive in a fast-paced environment, and enjoy working as part of a team. It’s not traditional therapy—it’s short-term, goal-oriented care that requires consistency, flexibility, and collaboration. Strong schedule adherence is key, as we rely on timely follow-ups and coordinated care. If you're looking for meaningful impact and are open to a different kind of mental health work, we’d love to connect.
Why MindHealthy?
At MindHealthy, we believe in caring for the caregiver. We support our clinicians with flexibility, respect, and meaningful work that makes a real impact.
What We Offer:
  • A stable Monday–Friday schedule
  • Work-from-home flexibility
  • Experts in CoCM who can guide and mentor
  • Support from peers who understand the job process and all that comes along with providing mental health care.

Benefits:
  • 401(k) 4% employer match
  • Health, dental, and vision insurance
  • Health savings account (HSA)
  • Life insurance
  • Paid time off (PTO)

Position Summary:
The Behavioral Care Manager(BCM) is one of three clinicians who collaborate for each patient. The BCM works in conjunction with the Primary Care Doctor and a Psychiatric Consultant who works for MindHealthy. The BCM will provide support through assessments, care planning, and regular follow-ups to help individuals improve their mental health. This includes using proven tools to track progress, adjusting care plans when needed, keeping accurate records, and working closely with our psychiatric team and other healthcare providers to ensure coordinated, effective care.
Essential Functions:
  • Help support the mental and physical health of a group of assigned patients.
  • Work with patients to understand their needs, make care plans, and connect them to helpful services.
  • Provide short-term mental health support using proven methods like motivational interviewing or problem-solving strategies.
  • Monitor patient progress through an internal EMR system and watch for any changes in their symptoms.
  • Work with the Psychiatric Consultant to support medication management, including checking on side effects and how well the treatment is working.
  • Join weekly meetings with the care team to review and update patient care plans.
  • Identify patients who may need more advanced mental health care and make sure they get the right support.
  • Handle other related duties as needed.

Competency:
  • Strong written and verbal communication skills.
  • Exhibit a positive, flexible and solution-focused attitude.
  • Ability to quickly synthesize medical and behavioral health data and formulate effective and evidence-based clinical recommendations.
  • Maintain active licensure within state(s) of jurisdiction.
  • Strong knowledge of clinical standards of care.

Experience / Education Requirements:
  • Completion of Master's degree in a mental health related field
  • Current unrestricted LCSW in NC, TX, FL, VA, or AR
  • Experience working with virtual patients.

Job Type: Full-time
License/Certification:
  • LCSW in NC, TX, FL, VA or AR
