What You’ll Do As a Patient Care Coordinator, you’ll play a vital role in our patients’ health journeys by conducting monthly telephonic outreach, assessing their unique needs, and creating individualized care plans. You will: • Manage patient care through scheduled phone conversations • Document visits using technology platforms and electronic health records (EHRs) • Develop care plans focused on physical, mental, and preventative health • Coach patients through their treatment plans—including wellness, nutrition, and goal setting • Help patients prepare for medical appointments and connect with resources What to Expect After You Apply At Wellbox, we believe in a thoughtful hiring process to ensure the best fit for both our team and our patients. Here’s what you can expect: • Timeline: The entire process typically takes 2–3 weeks from the date of application.

• Assessment: Qualified applicants will receive a timed assessment to evaluate computer literacy, basic math, and clinical knowledge. • Because this role is fully remote, strong tech skills are essential. • You must use a desktop or laptop to complete the assessment.

Tablets and phones will not work. • Virtual Interviews: Candidates who pass the assessment will move on to virtual interviews with our Recruiting Manager and Clinical Director. We look forward to learning more about you and how you can make a difference with Wellbox!

Requirements What We’re Looking For • Active Compact LPN license • At least 2 years of clinical experience (care coordination preferred) • Tech confidence: you’re comfortable using EMRs, Microsoft Office, and other digital tools • Strong communication and problem-solving skills Schedule & Compensation • Full-time, 40 hours/week | Monday–Friday between 8 AM – 6 PM in the patient\'s time zone • Orientation + Training (First 2 Months): $20/hr • Monthly Bonus Potential (up to $525) • Referral Bonuses: Up to $1,000
