Job description

POSITION: Care Partner

TITLE OF IMMEDIATE SUPERVISOR: Health and Wellbeing Director

DEPARTMENT: Resident Care

STATUS: Non-Exempt/Hourly

SUMMARY

This position will provide personal care service and engagement with residents and perform related services at the community in accordance with the plan of care assigned by the primary licensed nurse. This position reports directly to the Health and Wellbeing Director.


QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • High school diploma or GED
  • Successful completion of a state approved nursing assistant training program preferred
  • Current, active certification on the Georgia Nursing Assistant Registry preferred
  • Must successfully clear a background check, TB skin test or x-ray, and health screening
  • Current CPR certification
  • Current certificate of training for successful completion of the five (5) hour initial fire safety training provided by the Rules and Regulations of the Georgia Safety Fire Commissioner


PRINCIPLE DUTIES AND RESPONSIBILITIES

  • Provides assistance with assigned personal care tasks that meet resident physical and emotional personal needs consistent with the Orchard philosophy- For example: physical transfers, personal hygiene tasks such as mouth care, bathing and dressing, activity support, guidance and direction, and most importantly the ability to make a personal connection and engage respectfully, compassionately and meaningfully
  • Assists or leads small groups with engaging in one on one experiences with residents- Activities include socialization, personal growth, exercise, music, community integration, spiritual development, recreation and performance improvement under the direction of the Engagement Director
  • Responsible for assigned resident care tasks, ensuring accuracy, completeness and compliance with regulations, certification standards, legal and ethical standards
  • Resident care task includes but are not limited to maintaining personal hygiene, including bathing, oral care, and skin and nail care, hand washing, hair care, shaving, dressing, weighing, and bed making
  • Answers residents' call signals (Is the first responder to resident calls)
  • Assists with toileting, including bowel and bladder retraining and perineal care
  • Observes residents' conditions, measuring and recording food and liquid intake and output as needed and vital signs, and reports changes to professional staff
  • Provides resident care by applying dressings and supervising exercise routines
  • Provides residents help with walking, exercising, and moving in and out of bed
  • Performs and assists with mobility, transferring, ambulation, positioning and range of motion
  • Serves and collects food trays.
  • Uses appropriate and safe body mechanic techniques when lifting and/or transferring resident or supplies
  • Transports residents to and from dining room using a wheelchair when applicable
  • Uses aseptic technique for purpose of infection control; this includes good hand washing techniques and infection control policies
  • Answers phones, directs visitors, and delivers messages to residents and staff members maintaining confidentiality of all resident care information and assures residents' rights and privacy are protected at all times
  • Assists in orienting resident to environment and routines upon admission and throughout residency as needed
  • Documents accurately all care given, including dietary intake and output, if necessary; temperature, weight, and pulse
  • Reports to nurse or supervisor for written and verbal assignments, which include daily use of Personalize Service Plan for assigned residents before initiating care
  • Participates in the planning of resident care through shift reports and resident care staffing meetings and informs verbally and/or submit in writing any observations and changes in a resident for the licensed nurse to evaluate
  • Encourages/assists residents in keeping their rooms clean and orderly; also maintain nursing unit in a clean and orderly manner
  • Escorts residents to appointments and/or therapy departments as needed
  • Creates and maintains an atmosphere of warmth and personal interest as well as a calm environment
  • Provides a warm, welcoming and friendly atmosphere for residents, family, visitors and staff
  • Maintains the dignity of the resident
  • Provides person-centered care across treatment settings to improve the quality of life
  • Consults and collaborates with the interdisciplinary team and others involved in the resident's care
  • Focuses on solving conflict and not blaming
  • Listens to others without interrupting and keeps emotions under control- Remains open to others' ideas and tries new things
