Compensation & Benefits
- Base Salary: $80,000-$85,000
- Performance Bonus: $10,000 target; up to $15,000 annually
- Total Compensation Opportunity: $90,000-$100,000

Azalea Home Care, an established, growing multimillion-dollar agency with an 80+ person care team, is seeking a hands-on Director of Operations to own daily operations and build the infrastructure for our next stage of growth.
This is not a scheduling-only position. This leader will strengthen performance, retention, workflows, and care delivery with authority to resolve routine operational matters.
Key Responsibilities
- Own scheduling, staffing, caregiver operations, client service, intake-to-start-of-care, EVV, and billing and payroll readiness
- Oversee home-based and facility-based coverage, confirmations, call-out response, overtime, and client disruption
- Resolve coverage gaps, last-minute changes, and escalated client, family, or caregiver concerns
- Lead and hold the operations team accountable; support hiring, corrective action, and development
- Coordinate cross-department handoffs from intake and onboarding through care delivery, billing, and payroll
- Lead operating meetings, review KPIs, correct performance gaps, and enforce SOPs and escalation protocols
- Monitor labor utilization, documentation readiness, care quality, and operational financial performance
- Solve recurring problems and recommend the staffing, systems, and technology needed for growth
Performance & Bonus Measures
- 95%+ consistent shift coverage
- Fewer call-outs, no-shows, missed visits, and client disruptions
- Improved caregiver reliability, retention, and client service recovery
- Routine new clients starting within 48-72 hours after requirements are complete
- Timely handoffs, EVV exceptions, documentation, and billing readiness
- Controlled overtime and labor costs without compromising care
- Fewer routine issues requiring CEO involvement
Qualifications
- 3+ years of relevant experience required; 5+ years strongly preferred in home care, home health, healthcare operations, healthcare staffing, or private duty care
- At least 2 years leading employees, departments, or multiple operational functions preferred
- Experience supporting a multimillion-dollar organization or similarly complex operation preferred
- Strong knowledge of scheduling, staffing, intake, onboarding, EVV, billing, payroll, caregiver communication, and client service
- Proven success improving coverage, retention, start-of-care timelines, overtime, documentation, or efficiency
- Strong leadership, decision-making, conflict-resolution, communication, and accountability skills
- Experience using KPIs and implementing SOPs, workflows, handoffs, and performance systems
- Home care software, Medicaid, private duty care, facility staffing, or payer-documentation experience preferred
- Bachelor's degree in business, healthcare administration, operations, nursing, human resources, or a related field strongly preferred; MBA, MHA, or professional certification is a plus. Equivalent experience will be considered
- Hands-on, proactive, decisive, and steady under pressure
Compensation: $80,000.00 - $85,000.00 per year
