Work Environment

This role offers the flexibility of remote work while engaging with our hospice center's integrated support system. As a remote staff member, you will be part of a collaborative virtual team, participating in regular case conferences and training sessions to maintain high standards of service delivery. While primarily home-based, occasional in-person meetings may enhance team cohesion and client engagement, based on program requirements.

Key Responsibilities

• Conduct thorough assessments to identify clients' complex social, emotional, and environmental concerns, ensuring a detailed understanding of their unique situations.
• Develop and implement personalized service plans that address individual goals, outlining interventions and methods for progress tracking.
• Provide compassionate counseling and support to individuals and families facing end-of-life challenges, grief, and other significant life transitions.
• Work collaboratively with clients to identify their needs, connect them to community resources and support services, and empower them to advocate for themselves.
• Offer crisis intervention, including risk assessment and safety planning, to support clients in high-stress situations.
• Engage with relevant community agencies to coordinate services, ensuring a cohesive support network for clients, and advocating for their rights and access to necessary services.
• Participate in community outreach efforts to educate the public on hospice services and promote the agency’s resources.
• Attain ongoing professional development by actively participating in staff training and team meetings to stay updated on best practices in social work.

Patient Population / Client Focus

As an LCSW, you will work primarily with vulnerable individuals and families facing difficult circumstances, particularly within hospice care. Your support will focus on navigating issues related to terminal illness, grief, caregiving, and bereavement, ensuring that clients experience dignity and empowerment during their final stages of life. Your expertise will contribute to improved emotional and psychological outcomes for clients and their families.

Qualifications

• A Master's degree in Social Work from an accredited institution, ensuring a comprehensive understanding of clinical practices.
• Active state licensure as a Licensed Clinical Social Worker (LCSW), demonstrating your qualification to practice in Kansas.
• Strong knowledge of social and economic justice issues, trauma-informed care principles, and the dynamics of community resources.
• Exceptional communication skills, emphasizing cultural competence and an empathetic approach to client interactions.

Preferred Experience

Compensation and Benefits
