Hourly Compensation + Shift Differentials:
Weekday 2nd: +$3.00 per hour
Weekday 3rd: +$3.00 per hour
Weekend 1st: +$1.00 per hour
Weekend 2nd: +$4.00 per hour
Weekend 3rd: +4.00 per hour

Essential Job Duties:
Documentation amp; Assessment
  • Completes all required clinical documentation independently in accordance with VMP policies, including assessment and documentation of residents’ physical, psychological, social, and spiritual status.
Care Coordination amp; Communication
  • Coordinates and communicates effectively with physicians, interdisciplinary team members, residents, and responsible parties to ensure safe, timely, and appropriate care delivery.
Clinical Leadership amp; Supervision
  • Directs, supervises, and evaluates the performance of clinical staff; models safe nursing practice and ensures adherence to professional standards and facility policies.
Clinical Judgment amp; Emergency Response
  • Identifies and initiates appropriate action in routine, urgent, and emergency situations; manages conflict using a direct, professional approach; and reprioritizes care to maintain continuity of resident services.
Nursing Process amp; Care Planning
  • Completes admission, ongoing, and change-in-condition assessments; initiates and updates nursing diagnoses and individualized care plans based on MDS data, medication review, and other clinical information.
Clinical Knowledge amp; Decision-Making
  • Applies knowledge of pathophysiology and disease processes to assess resident conditions, implement nursing interventions, and evaluate outcomes.
Direct Patient Care
  • Provides direct nursing care efficiently and effectively, addressing residents’ physical, psychosocial, and spiritual needs in accordance with the nursing process and established plans of care.
Medication Administration
  • Administers medications accurately and timely in compliance with VMP policies and procedures; documents medications at the time of administration; and notifies the physician and responsible parties of significant changes in resident condition.
Professional Practice amp; Geriatric Care
  • Participates in staff orientation, performance evaluations, and facility committees; serves as a clinical resource and educator to staff; and delivers compassionate, patient-centered care to geriatric residents, including those with cognitive impairment.
Licensure, Education amp; Training
  • Maintains active Wisconsin RN licensure; completes a minimum of 12 CEUs annually; and successfully completes all mandatory in-house training, skills reviews, and competency testing.
Education Requirements:
  • An active, valid RN license with the State of Wisconsin
