Job description

CareHarmony works comprehensively with providers to deliver value-based care management initiatives for their patients. Under the direction of the department manager, the Medical Assistant will be assisting the team in referral management for High-Risk patients and helping to assist with patient needs.

Requirements

Responsibilities/Day in the Life:

  • Receive new patient referrals from clients
  • Create charts for all new patients
  • Input medical information in new chart
  • Help Care Coordinators with patient needs as applicable
  • Foster relationships with their team to ensure the CareHarmony culture is positive and supportive.
  • Other duties as applies

Physical Requirements

  • This position is sedentary and will require sitting for long periods of time
  • This position will require the ability to speak clearly and listen attentively, often by telephone, for an extended period of time 
  • The position will require the ability to understand, process, and take thorough notes in real-time on telephone conversations
