• Maintain up-to-date and accurate clinical documentation, including progress notes and treatment reviews, in accordance with departmental policy and regulatory standards.
• Collaborate with an interdisciplinary healthcare team to develop holistic treatment plans tailored to each patient's psychosocial assessments.
• Engage with clients' families or support systems to coordinate interventions and discharge plans, ensuring a smooth transition for patients upon their program completion.
• Create and implement individualized treatment plans that address specific patient needs and circumstances, adjusting approaches as necessary based on ongoing assessments.
• Conduct regular counseling sessions with clients, actively monitoring their progress and adapting treatment methodologies to optimize outcomes.
• Perform continuous assessments of client progress and work effectively within electronic health record systems to track outcomes and document necessary changes to treatment plans.

• Prior experience in hospice care or similar settings is preferred.
• Demonstrated competency in using electronic health records and telehealth platforms for documentation and clinical supervision.
• Experience providing group therapy or leading support groups would be advantageous.
