Position Overview

Berks Community Health Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team. This full-time remote position is integral to our Detox Center, where you will provide essential clinical services aimed at improving the mental health and wellbeing of our patients. Work Environment

This role is fully remote, allowing for flexibility while working within our Detox Center framework. Although the position is performed from a home office, you will regularly collaborate with a multidisciplinary team, including medical staff, therapists, and case managers, through virtual meetings. Key Responsibilities
  • Conduct thorough assessments and develop tailored treatment plans for individuals, ensuring that therapeutic interventions align with their unique needs and background.
  • Provide both individual and group counseling sessions, employing evidence-based approaches such as cognitive behavioral therapy (CBT), motivational interviewing, and relaxation training to foster healing and resilience.
  • Perform crisis intervention and triage as necessary, demonstrating swift and effective support for patients in acute distress.
  • Manage case responsibilities, including documentation and follow-ups for referrals, ensuring comprehensive network support for clients.
  • Collaborate with the wider team during multidisciplinary meetings to discuss case consultations, treatment progress, and strategies for improved patient care.
  • Engage in outreach initiatives focused on mental wellness, educating the community about available resources and promoting healthy coping strategies.
Patient Population / Client Focus

The primary focus of this role will be on patients in detoxification treatment, many of whom are facing issues related to substance use and its psychological impacts. Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • Must possess an active license to practice as a Licensed Clinical Social Worker (LCSW) in the state of Pennsylvania.
  • A minimum of three years of experience working in the behavioral health field, preferably within addiction recovery services.
  • Proficiency in using computer systems, office software, and telehealth platforms essential for managing documentation and patient interactions.
