Array Wellness Center PC is seeking a compassionate and skilled Licensed Clinical Social Worker (LCSW) to join our dedicated team in providing essential support to vulnerable populations. Work Environment

Key Responsibilities
  • Conduct detailed assessments to understand and identify the unique social, emotional, and environmental challenges faced by clients.
  • Develop and implement personalized service plans that detail specific goals, interventions, and methods for monitoring progress.
  • Provide individual and family counseling support, guiding clients through a variety of psychosocial difficulties.
  • Guide clients in connecting with the necessary community resources to ensure comprehensive support.
  • Engage in crisis intervention strategies, including risk assessments and safety planning, particularly in high-stress situations.
  • Actively collaborate with various community organizations to coordinate services and advocate for client needs effectively.
  • Participate in outreach activities to increase awareness of our agencyโ€™s services within the community.
  • Attend case conferences and training sessions to contribute to team learning and development.
Patient Population / Client Focus

Your primary focus will be on individuals and families confronting significant life challenges often associated with hospice care, requiring a sensitive understanding of end-of-life issues, grief, and coping strategies. You will also address broader social issues that affect these populations, such as emotional distress and socioeconomic constraints, ensuring clients are supported holistically.

Qualifications
  • Master's degree in Social Work from an accredited institution.
  • Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
  • Solid understanding of issues surrounding social and economic justice, trauma-informed care, and community resources.
  • Proven communication skills and cultural competence, enabling effective engagement with a diverse client base.
Preferred Experience
  • Previous experience working in a social service agency or similar setting.
  • Familiarity with hospice care and end-of-life counseling is highly beneficial.
  • Experience using electronic health records (EHR) systems to document client interactions and treatment plans.
Compensation and Benefits
