Mileage Reimbursement | Mondayโ€“Friday Schedule

Make a Difference in Your Community

We are seeking a compassionate and detail-oriented Registered Nurse (RN) to join our team as a Long-Term Services and Supports (LTSS) Screener. In this vital role, you will conduct comprehensive assessments to determine eligibility for Medicaid-funded long-term care services, helping vulnerable individuals access the support they need to remain safe and healthy.

This position offers a rewarding opportunity to work independently while collaborating with community partners and the Department of Social Services (DSS) to improve outcomes for individuals across the Rappahannock-Rapidan Health District.

Service Area

Base Location: Fauquier County

This is a field-based position serving the following counties:

  • Culpeper
  • Fauquier
  • Madison
  • Orange
  • Rappahannock

Travel throughout the district is required. Mileage reimbursement is provided.

Key Responsibilities

  • Conduct face-to-face LTSS screenings in participants' homes.
  • Complete comprehensive nursing assessments and the Universal Assessment Instrument (UAI).
  • Determine medical necessity and eligibility for Medicaid-funded long-term services and supports.
  • Collaborate with Department of Social Services staff during joint assessments and screenings.
  • Maintain accurate, timely, and compliant documentation.
  • Authorize appropriate waiver services based on assessment findings.
  • Participate in appeals related to denied waiver services when required.
  • Maintain current knowledge of home and community-based service programs.
  • Meet productivity, quality, and timeliness standards.
  • Demonstrate professionalism, punctuality, and strong customer service skills.
  • Perform other duties as assigned.

Qualifications

Required
  • Active, unrestricted Virginia RN License
  • Current BLS/CPR Certification
  • Strong clinical assessment and critical thinking skills
  • Excellent organizational and time management abilities
  • Proficiency with computers and electronic documentation
  • Ability to successfully complete LTSS Certification Training
  • Reliable transportation and willingness to travel throughout the district
  • Ability to work independently and manage a field-based caseload

Preferred Qualifications

  • Previous LTSS Screening experience
  • Home Health or Home Visiting experience
  • 1โ€“3 years of Case Management experience
  • Bachelor's Degree in Nursing (BSN)
  • Experience working with Medicaid-funded programs and community-based services
