Provide personal care under the supervision of a nurse according to the patient's plan of care. Collaborate with transdisciplinary team in a variety of settings. This is a weekend only position, 12 hours, Saturday/Sunday. This will support our West Campus team based out of our Fairview Park, Ohio office.

 

WHAT YOU WILL DO: 

  • Provide personal care and assist with ADL's according to the POC.
  • Provide safe environment for patients.
  • Safely ambulate/transfer patient as appropriate.
  • Assist with passive and active ROM exercises as directed by the nurse.
  • Prepare nutritional snacks as appropriate for the patient.
  • Under the supervision of the nurse, assist patient with pre-poured medications that are ordinarily self-administered and that are prescribed by a physician under the     supervision nurse (Home Care only).
  • Under the direction of the nurse, perform simple wound care, skin care, and bowel care protocols according to the plan of care where appropriate.
  • Recognize and promptly report any changes in the mental or physical status of the patient to the nurse.
  • Record in clinical record, on appropriate documents, the activities provided and observations of patient at the time of visit.
  • Complete all charting, timesheets and mileage logs as defined by the agency policy and procedures.
  • Attend and actively contribute during patient care conference.
  • Participate in HNA discipline meetings, team meetings, staff meetings, retreats and in-services as appropriate.
  • Attend in-services to obtain 3 education hours per quarter.
  • Participate in the on-call rotation as scheduled (Home Care only).
  • Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.)
