Position Summary

The Weekend RN Clinical Manager serves as the clinical and operational leader for Home Health services during weekend operations. This role ensures the delivery of high-quality, safe, and compliant patient care while providing leadership, guidance, and support to clinical staff across the region.

Working closely with the Director of Home Health and interdisciplinary teams, the Weekend Clinical Manager oversees patient care coordination, staff support, regulatory compliance, quality improvement initiatives, and operational decision-making to ensure seamless care delivery throughout the weekend.

This position plays a vital role in maintaining exceptional patient outcomes, supporting staff success, and ensuring continuity of care when patients and clinicians need leadership the most.


What You'll DoClinical Leadership & Patient Care
  • Provide clinical oversight and leadership for weekend Home Health operations.

  • Support clinicians in complex patient care situations and clinical decision-making.

  • Oversee the development, implementation, and monitoring of individualized plans of care.

  • Ensure safe, effective, and patient-centered care delivery.

  • Conduct home visits as needed to support patient care and staff.

  • Assist with care coordination, transitions of care, and discharge planning.

  • Collaborate with physicians, hospitals, referral partners, and community resources.

Staff Leadership & Development
  • Provide supervision, coaching, mentorship, and support to Home Health clinicians.

  • Foster a culture of accountability, teamwork, engagement, and professional growth.

  • Participate in staff onboarding, orientation, competency validation, and performance evaluations.

  • Support recruitment, retention, and employee engagement efforts.

  • Recognize and celebrate team accomplishments and professional development.

Quality & Compliance
  • Ensure compliance with Medicare Conditions of Participation, OASIS requirements, CMS regulations, ACHC standards, and organizational policies.

  • Review documentation for timeliness, accuracy, and regulatory compliance.

  • Conduct chart audits and monitor quality indicators and Value-Based Care metrics.

  • Support quality improvement initiatives and corrective action planning when needed.

  • Promote best practices that drive exceptional patient outcomes and satisfaction.

Operations & Weekend Coverage
  • Coordinate patient scheduling, staffing coverage, and resource allocation.

  • Collaborate with scheduling and operational teams to ensure continuity of care.

  • Monitor operational reports and clinical performance indicators.

  • Participate in the Clinical Manager on-call rotation.

  • Provide scheduled on-call leadership coverage from Saturday morning through Monday morning.


What You'll BringEducation & Licensure
  • Graduate of an accredited school of nursing meeting NLN educational standards

  • Associate Degree in Nursing required; BSN preferred

  • Current, unrestricted Registered Nurse (RN) license in Maine

  • Current Basic Life Support (BLS) certification

Experience
  • 3–4 years of progressive nursing experience in acute care required

  • Home Health experience required

  • Minimum of 2 years of supervisory experience required

  • Home Health leadership experience preferred

Knowledge & Skills
  • Strong knowledge of Home Health regulations, Medicare Conditions of Participation, OASIS, CMS requirements, and Value-Based Care principles

  • Exceptional clinical assessment and critical thinking skills

  • Proven leadership, coaching, and staff development abilities

  • Experience managing complex patient care situations

  • Knowledge of quality improvement methodologies and regulatory compliance

  • Strong communication and relationship-building skills

  • Ability to prioritize and manage multiple responsibilities in a dynamic environment
