The Role:TheDietitian Nutritionist, Senior Coordinatorworks withclientsreferred to us by our insurance partners who provide Medically Tailored Services.This program type requires the clients to receive a nutrition assessment at least once every six months.TheDietitian Nutritionistwill provide nutrition assessments, set goals with the client,and offer the individual any of the other services available through Project Bread.TheDietitian Nutritionistprovides ongoing case management support for each client throughout their enrollment in ourprogram.

Successful candidates willhavepreviousexperience providing nutrition counseling to individuals in low-income communities.They must be able to connect with clients from diverse backgrounds with a variety of health conditions to set incremental goals for sustainable change.

AboutHealth-Related Social Needs

The Commonwealth of Massachusettshasa waiver from the Centers for Medicaid and Medicare to improve the ways in which MassHealth provides care to its members, which includesprovision of supportsfor health-relatedsocial needs such asnutrition and housing.

Project Bread is an approved nutrition servicesprovider fortenaccountable care organizations.In this role we receive referrals and provide nutritional services to members fromover 100health centers, hospitals, and physician offices across Massachusetts.

Primary Responsibilities:

  • Nutrition Education:
    • Conduct nutrition assessments with clients and help them set incremental goals to improve their health.
    • Design nutritional service plans to meet the needs of each client, this may include:
      • provision of foodvouchers;
      • procurement ofhousehold kitchensupplies;
      • enrollment in Project Bread cookingclasses;
      • ongoing nutritioncounseling;and
      • connections to SNAP, WIC, School Meals, and otherfoodsassistanceprograms.
  • Caseload Management:
    • Complete intake assessments withclients toidentifytheir individual barriers to healthy food access.
    • Buildrelationshipswith clientsandensure that theservices provided meettheir needs.
    • Conduct follow-up callsand eligibility screeningwithclientson an ongoing basis.
  • LanguageCapabilities(Required)
    • Conduct assessments and provide ongoing client support via phone,textsand emailsinSpanish.
    • Interpretand/orteachcooking classes.
    • Review translated materials for cultural and contextual relevance.
  • Additional Program Coordination Support:
    • Maintainaccurateclientrecords in Salesforce.
    • Develops andmaintainsongoingrelationshipswithreferringhealth center staff to ensureoptimalsupport for each member.
    • Promote usage of Project BreadsFoodSourceHotline.
    • Perform other related tasks as needed.

Organizational Responsibilities:

  • Participate in the annual Walk for Hunger, including working as eventstaffthe weekend of the Walk (first weekend in May), and attending pre-Walk events as assigned.

  • Priorexperience providing direct services to low-income individuals, preferably in nutrition, client service, and/or health care.

Work Schedule

All employees are currentlyworking ona hybrid remote and in-office model and are expected to be present in the East Boston office on a regular basis.Remote work schedules aredeterminedby departmentsand can vary across teams. TheDietitian Nutritionist,SeniorCoordinatoris required towork from the officeon Wednesdays.TheDietitian Nutritionistmay also need to work fromtheofficeadditionaldays based on their meeting schedule.

This position occasionally requires availability until 6:30pm for counseling or cooking classes.This position also requiresflexibility in scheduling counseling sessions withclients.This includes making 8am and 6pm sessions available once a week.Coordinators can end early or start late on the days when they offer these outside of business hours sessions.
