Job Title: Registered Nurse (RN) – Home Health & Hospice

Location: St. Johnsbury, Vermont

Pay Rate: $2,500 to $2,720/Week | $68.00/Hour

Contract: 12 Weeks

Schedule: Day Shift | Monday–Friday | Possible Weekend Shifts

Experience: Minimum 1 Year of Clinical Nursing Experience Required

About the Role

We are seeking a compassionate Registered Nurse (RN) for a 12-week Home Health & Hospice contract opportunity in St. Johnsbury, Vermont. This role involves delivering skilled nursing care in home health and hospice settings, coordinating individualized care plans, and collaborating with an interdisciplinary team to support quality patient outcomes.

Responsibilities
  • Provide comprehensive nursing care, including patient admissions, assessments, and ongoing evaluations.

  • Develop and implement individualized care plans for home health and hospice patients.

  • Collaborate with physicians, therapists, and interdisciplinary team members to coordinate patient care.

  • Educate patients and families on treatment plans, disease management, and home care needs.

  • Complete accurate clinical documentation using Homecare Homebase (HCHB) while maintaining compliance with care standards.

Requirements
  • Active Vermont Registered Nurse (RN) license required.

  • Minimum 1 year of clinical nursing experience required.

  • Home Health and/or Hospice nursing experience preferred.

  • Experience with Homecare Homebase (HCHB) EMR preferred.

  • Availability to work day shifts with possible weekend coverage.
