Position Overview

Seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing services to patients throughout Marion, Hamilton, and Johnson counties. This role focuses heavily on OASIS documentation, including Start of Care (SOC), Recertification, and Discharge assessments, while delivering high-quality patient care in the home setting. The ideal candidate will have previous home health experience, proficiency with Homecare Homebase (HCHB), and a commitment to providing compassionate, patient-centered care.

Schedule & Work Details

  • Coverage Areas: Marion, Hamilton, and Johnson Counties, IN
  • Schedule: Monday through Friday
  • Shift: 8:00 AM - 4:30 PM
  • On-Call/Weekend Requirement: None
  • Mileage Reimbursement: Yes
  • Dress Code: Solid-color scrubs (any color)
  • Pay Rate: $51.00 - $53.00 per hour

Key Responsibilities

  • Complete OASIS Start of Care (SOC) assessments accurately and in accordance with Medicare and agency guidelines.
  • Perform Recertification assessments and update patient plans of care based on clinical findings.
  • Complete Discharge assessments and documentation, ensuring continuity of care and appropriate patient transitions.
  • Conduct comprehensive nursing assessments in the patient's home.
  • Provide skilled nursing interventions, including medication setup, medication management, and patient education.
  • Perform wound assessments, wound care treatments, and ongoing monitoring of wound healing progress.
  • Develop, implement, and revise individualized care plans in collaboration with physicians and interdisciplinary team members.
  • Educate patients and caregivers regarding diagnoses, medications, treatment plans, disease management, and home safety.
  • Monitor patient progress and communicate changes in condition to physicians and other care team members.
  • Maintain accurate, timely, and compliant clinical documentation within Homecare Homebase (HCHB).
  • Coordinate care with physicians, therapists, social workers, and other healthcare professionals to promote positive patient outcomes.
  • Manage a daily patient visit schedule across the assigned coverage area.

Required Qualifications

  • Active and unrestricted Registered Nurse (RN) license.
  • Previous home health experience required.
  • Experience with Homecare Homebase (HCHB) required.
  • Experience completing OASIS assessments and documentation.
  • Strong clinical assessment and critical thinking skills.
  • Experience with medication management and wound care.
  • Valid driver's license, reliable transportation, and ability to travel throughout the service area.
  • Excellent communication, organization, and documentation skills.

Preferred Qualifications

  • Extensive experience with OASIS SOC, Recertification, and Discharge assessments.
  • Strong knowledge of Medicare home health regulations and compliance standards.
  • Exceptional communication and interpersonal skills.
  • Positive attitude and patient-focused approach to care.
  • Ability to work independently while effectively managing a field-based caseload.
  • Experience collaborating with interdisciplinary healthcare teams.
