Job description

Summary: The Case Manager provides skilled nursing care to patients in their homes in accordance with the agency’s policies, state of Wisconsin and federal regulations, and the individualized plan of care, including supervision of PCW to ensure quality assurance and implementation of care plan. The RN delivers high-quality clinical care, coordinates services, and supports patients and families in achieving optimal health outcomes. Bilingual ability is considered a strong asset to better serve diverse patient populations.

Key Responsibilities (Skilled Nursing):

Clinical Care • Perform comprehensive patient assessments, including physical, psychosocial, and environmental evaluations. • Develop, implement, and update individualized plans of care in collaboration with the interdisciplinary team. • Provide skilled nursing services such as wound care, medication management, IV therapy, catheter care, chronic disease management, and patient education. • Monitor patient progress, document clinical findings, and communicate changes to the physician and care team. • Ensure all care is delivered in compliance with state regulations, agency policies, and professional standards.

Care Coordination • Collaborate with physicians, therapists, social workers, and other healthcare professionals to ensure continuity of care. • Participate in case conferences and contribute to care planning discussions. • Educate patients and caregivers on disease processes, medications, safety, and self-management strategies.

Personal Care Services PCST Assessment & Charting • Conduct comprehensive PCST assessments to determine client eligibility and service hours based on functional limitations and medical necessity. • Complete accurate PCST charting, including ADL scoring, frequency/level of assistance, and clinical justification. • Document all findings in accordance with state Medicaid guidelines and agency policy. • Update PCST documentation when client condition changes or during required reassessment periods.

Care Planning & Clinical Oversight • Develop individualized PCS care plans aligned with PCST results and client needs. • Review and revise care plans as needed to reflect changes in client condition or service requirements. • Provide supervisory visits to PCWs to ensure care plan adherence and quality of care. • Offer clinical guidance and support to PCWs regarding safe and effective personal care techniques

Client Care & Monitoring • Perform nursing assessments, monitor client health status, and identify changes requiring intervention. • Communicate significant findings to physicians, case managers, and interdisciplinary team members. • Educate clients and families on safety, chronic disease management, and personal care strategies. • Promote client independence, dignity, and safety in the home environment.

Regulatory & Documentation • Complete timely and accurate documentation in the electronic medical record (EMR). • Maintain compliance with state, federal, and accreditation standards. • Participate in quality improvement initiatives and agency audits as needed. • Ensure all PCS and PCST documentation meets state, federal, and agency compliance standards. • Participate in audits, quality assurance reviews, and performance improvement initiatives. • Follow HIPAA and confidentiality guidelines at all times.

Professionalism & Communication • Demonstrate cultural sensitivity and effective communication with patients, families, and team members. • Utilize bilingual skills (if applicable) to enhance communication and improve patient outcomes. • Maintain professional boundaries, confidentiality, and ethical standards

Qualifications • Active and unrestricted Registered Nurse (RN) license in the state of Wisconsin. • Minimum of one year of clinical nursing experience; home health experience preferred. • Strong assessment, critical-thinking, and clinical decision-making skills. • Valid driver’s license, reliable transportation, and proof of auto insurance. • Proficiency in EMR (OASIS) documentation.

Work Schedule • Full-time position with flexibility to meet patient needs. • May include occasional weekends or on-call rotation depending on agency requirements.

Physical Requirements • Ability to travel to patient homes. • Ability to lift, move, or assist patients as needed. • Ability to stand, walk, or sit for extended periods. • Demonstrated ability to operate and train Personal Care Workers (PCWs) in the safe use of a Hoyer lift
