Job description

The RN Visit Nurse is responsible to plan, organize and direct home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Position: RN or LPN Visit Nurse for Home Health
RN Salary: $85,000-$87,000/year, depending on experience
LPN Hourly: $23.00/hour - $26.00/hour, depending on experience for training and then PPV of $45 per completed visit

Service Area: Northeast - Saint Paul, Stillwater, Arden Hills, Circle Pines, Hugo. Plus, mileage reimbursement of 72.5 cents per mile.

Schedule: 1.0 FTE, Monday through Friday 8am-5pm (RN: weekend, on-call and holiday rotation will apply / LPN: no on-call, every 6th weekend and 2 holidays per year)

Position Functions & Responsibilities:

  • Completes an initial assessment of client and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Regularly re-evaluates client nursing needs.
  • Initiates the plan of care and makes necessary revisions as client status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the client and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct client care as defined in the State Nurse Practice Act.
  • Counsels the client and family in meeting nursing and related needs.
  • Provides health care instructions to the client as appropriate per assessment and plan of care.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client.
  • Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
  • Lead by example in establishing rapport with clients, clinicians, partners, and the community.
  • Ensure positive/professional communication within the team
  • Customer service resolution; escalate concerns
  • Prepares clinical notes and updates the primary physician when necessary and at least every 60 days.
  • Communicates with the physician regarding the client’s needs and reports any changes in the client’s condition; obtains/receives physician’s orders as required.
  • Communicates with community health related persons to coordinate the care plan.
  • Timely completion of all documentation as it relates to the care plan by 10am the next day
  • Understands and complies with all the rules and regulations that govern the responsibilities of a registered nurse
  • Completes orientation and annual training requirements as assigned
  • Participates in Lifespark’s Event Management process including initiating reports, escalation of events and investigations
  • Promotes safety in the workplace through real time coaching and participation in safety awareness training
  • Instructs, supervises, and evaluates home health aide care provided every two (2) weeks
  • Manages client supplies assuring appropriate DME provider is utilized.
  • Schedules and keeps member appointments
  • Organizes travel efficiently and documents mileage accurately within EMR
  • Other duties as assigned

Qualifications:

  • One to two years of recent acute care experience in an institutional setting. One year of home health care experience preferred.
  • Experience in customer service/direct member contact field preferred
  • Experience supervising caregivers
  • Graduate of an accredited school of nursing
  • Current Minnesota Registered Nurse (RN) or Licensed Practical Nurse (LPN) License
  • Must pass a MVR check based on Lifespark company policy
  • In accordance with CMS Vaccine Mandate, candidate must provide proof of Covid-19 Vaccine or submit documentation for exemption
