EMPLOYER: Pediatric Nursing Specialists of Indiana (PNS), a Home Care Network (HCN) agency.

JOB TITLE: PRN RN Case Manager - Home Health

  • This is a PRN RNCM leadership position with our agency with long-term employment potential and opportunities for quick growth into a Part-Time, Contractor, or Full-Time RNCM position as our agency grows over the next 6-12 months.
  • This role requires RNCM services, care, and travel as needed to all clients throughout Indiana.
    • This role works closely with and reports directly to our Indiana Regional Program Administrator (local) and Director of Clinical & Administrative Operations (hybrid, based in Ohio).
    • For those pursuing career growth: a defined path for advancement to Full-Time RNCM, Assistant Program Administrator, Program Administrator, Clinical Administrator, Director of Nursing (DON or Asst. DON), Business Administrator, QA / Compliance leadership, and other positions will be supported.
  • Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall.
  • At least 1 year of experience is required in: Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
  • At least 1 year of paid experience is required at/with a home health agency or in-home care provider (or equivalent); 2-5 years' experience is preferred.
  • Desirable but not required:
    • Nurse Supervision experience
    • People management
    • Peer/staff education and development
    • Success or willingness to build relationships with client payors, case mgr / entity relationships (i.e., AAA, DoDD County Board, VA, etc.) in NW Ohio and our current + prospective employees in these communities.
  • This role requires a mix of case management, recertifications, starts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews.
  • At least 1 year of Case Management experience and home health agency (or equivalent) experience are required.
    • Must have experience with OASIS and NON-OASIS assessments.
    • 1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus.
    • Experience and willingness to serve clients of all ages (infants, pediatrics, adults, seniors)

OPTIONAL: TEMPORARY / BONUS ARRANGEMENT

  • To support our agency's clients, staff, and business throughout our Clinical Director's upcoming maternity leave, we are motivated to create a custom agreement with the right RN (experienced, willing, and reliably available) to:
    • Report 1-3 days per week (Mon-Wed) to our Indianapolis Office (8580 Cedar Place Dr, Indianapolis, IN 46240) and/or as needed during State Surveys
    • Provide occasional on-call and varied / flex hour coverage (late night / weekend emergency) as needed
  • Compensation, terms, and responsibilities would be in addition to all listed here and formally agreed-to prior to start.

EMPLOYMENT TYPE: All capacities available - Part-Time, Full-Time, PRN, Contractor, or Hybrid

  • PNS/HCN will tailor your schedule and employment based on your interest in a Part-Time, Full-Time, PRN or Contractor commitment.
  • No On-Call, Overnight, or Overtime shifts required!
    • Additional and OT visits may be available and offered if desired.
  • Weekend, Overtime, Respite & other Nursing PRN services are offered when available but are not required to accept.

SERVICE AREA: Indianapolis, IN and surrounding 2-hour radius

  • Primary service area is centralized in Indianapolis and Indianapolis Metro.
  • HCN requires typical travel distances in a 10-60 minute radius, and occasional travel up to 2 hours. *Additional coverage areas are available for extra hours if desired (not required).

JOB LOCATION OPTIONS:

  • Marion County, Indiana is central to the clients in this service area, but qualified applicants residing outside of but near Marion County will be considered so long as travel requirements throughout our service areas are fulfilled.
  • Our agency's Indiana Office is located at 8580 Cedar Place Dr, Indianapolis, IN 46240 (in the Nora / Far Northside of Indianapolis, near Keystone at the Crossing) and is available to RNCMs as needed.

SCHEDULE TYPE: Primarily Weekdays/Daytime (M-F / 8am - 5pm) with possible early evening and rare Weekend shifts available.

  • Flexible and customizable. We'll work around your other employment and/or personal schedules, but will require most of your availability to be Weekdays, Daytime.
    • RNs with nights-only and weekends-only availability cannot be considered at this time.
  • We prioritize visits as close to your home as possible - that fits your availability and preferences.

Job Summary
As an RN Case Manager, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes.

  • With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.

Responsibilities/Requirements of an RN Case Manager / Clinical Nurse Supervisor include:

  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
  • Minimum of one (1) years of nursing supervisory experience. Home care experience preferred.

Qualifications

  • Graduate of an accredited School of Nursing.
  • Minimum of one (1) year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Indiana.
  • Minimum of one (1) year of experience in a home health or acute care setting or equivalent experience. 1+ years of paid home health / in-home care experience is also required. 2+ years of both are preferred.
  • Minimum of one (1) year Experience in case management is required.
  • CPR and First Aide Certifications required (existing or can be obtained prior to hire date through our agency or accredited third parties).
  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.
