Job description

Position Summary
The Case Manager for the State Opioid Response (SOR) Rapid Rehousing Program provides outreach, assessment, and housing support to individuals transitioning from residential substance use treatment into independent living. This position involves building strong community relationships, supporting tenancy stabilization, and providing case management with a trauma-informed, person-centered approach. The Case Manager assists participants with identifying safe, affordable housing, accessing benefits, and integrating into the community.
Pay Range: $19-$21 an hour
Open Shift: Monday through Friday 9am-5pm (40hrs), some flexibility required
Key Responsibilities
  • Assess participant eligibility and support enrollment in the Rapid Rehousing program.
  • Conduct community-based outreach and engagement to identify participants' strengths, needs, and recovery goals.
  • Assist participants in locating affordable housing, including applications, landlord meetings, and apartment viewings.
  • Support participants transitioning from treatment to independent living, including after housing placement.
  • Provide tenancy support and education, assisting participants with landlord communication and housing responsibilities.
  • Help participants develop safety plans and relapse prevention strategies.
  • Build and maintain strong working relationships with local landlords and housing providers.
  • Manage participant files, documentation, and service notes in accordance with program guidelines.
  • Coordinate move-in logistics, including furnishings, utilities, and address changes.
  • Assist with employment and education efforts to support long-term housing stability.
  • Support participants with budgeting, money management, and benefits navigation.
  • Provide transportation to appointments and housing-related activities as needed.

The Division of Supportive Housing (DSH) provides safe, stable housing and individualized wraparound services for individuals who have experienced chronic homelessness, many of whom are also navigating challenges related to substance use, incarceration, or trauma. DSH programs focus on long-term stability and help participants transition to independent, permanent housing. Through person-centered care, housing navigation, and community-based supports, we empower individuals to build a foundation for recovery, self-sufficiency, and a better quality of life.

Requirements
  • Valid driver's license, acceptable driving record, reliable vehicle, and sufficient automobile insurance.
  • Demonstrated ability to work independently, manage time, prioritize tasks, take initiative, follow through, and work cooperatively as a member of a team.
  • Must be able to pass and maintain certifications in CPR and first aid.
  • Must be at least 21 years old.
  • Requires 6 months experience with individuals with mental health and/or substance use challenges or individuals with a history of housing instability preferred.
  • High school diploma or GED required. Bachelor's degree preferred.
