Position: Case Manager | Vogue Recovery AZ | Full-Time |

Monday - Thursday 1:00pm - 11:30pm or Saturday - Monday 9:00am - 10:30pm

We are looking for a dynamic Case Manager to support clinical, non-clinical, and medical staff in the provision of technical and auxiliary clinical services to ensure comprehensive and effective daily programming and functionality. Specific responsibilities include conducting initial assessments, group counseling, case management services, and aftercare planning.

Job Description- Case Manager Job Functions:

  • Monitor and document client progress throughout the course of treatment
  • Engage with clients upon admission and maintain consistent support through discharge
  • Provide individualized support under the direction of the primary therapist and clinical team
  • Identify client needs and communicate pertinent information to the treatment team
  • Assist clients with:
  • Financial responsibilities (e.g., bill payments)
  • Family coordination and communication
  • Educational and vocational concerns
  • Legal appointments and court dates
  • Referrals to sober living, outpatient, or step-down programs
  • Housing, transportation planning, and employment resources
  • Conduct weekly check-ins with IOP clients to assess progress and identify needs
  • Schedule and coordinate IOP-related services including therapy, medical, psychiatric, and ancillary appointments
  • Maintain communication with IOP providers to ensure client participation and progress
  • Support clients transitioning into or out of IOP care, including warm handoffs to new providers
  • Work with residential clients to address ongoing outside needs (appointments, court dates, family issues, schooling, etc.)
  • Coordinate and track all residential client appointments; ensure appropriate staff are informed
  • Assist with discharge planning in collaboration with the clinical team
  • Provide case management for overflow detox clients as assigned
  • Conduct initial needs assessments for detox clients and coordinate immediate services (e.g., medical follow-up, family communication)
  • Ensure detox clients receive appropriate information and referrals as they transition into the next level of care
  • Coordinate and manage all case management services for each client, ensuring consistency and follow-through
  • Maintain timely and accurate documentation of all case management interactions
  • Participate in weekly clinical team meetings and receive oversight from the Clinical Director
  • Facilitate psycho-educational groups as needed
  • Primarily responsible for the processes related to the pre-admission, intake, and admission of new clients.
  • Will be available as needed to retrieve new clients from airport and provide intake.
  • Will design and improve quality assurance, patient experience, and performance measures.
  • Will coordinate with families of patients to arrange for supplies, funds, or resources for clients.
  • Will provide support for families of patients in the form of advice, resources, and referrals.

Requirements:

  • HS Diploma
  • A valid driver’s license and a clean driving record
  • Fingerprint Clearance Card
  • Strong interpersonal skills
  • Case Management experience
  • Knowledge of all Microsoft office suites including, but not limited to: Word, Excel, PowerPoint, Outlook etc.
  • Versed in the use of Electronic Medical Records
  • Clinical knowledge pertaining to substance use disorders

Preferred Qualifications:

  • Experience and knowledge in the field of Substance Use Disorders
  • Possess or in process of attaining CADC/LCADC
  • Versatility, flexibility, and willingness to work with constantly changing priorities
