RN or LPN Clinical Liaison Home Health/Hospice, VNA Care - Major Boston Hospital Job Share

This position is full-time on site at a Boston Hospital 5 days per week

Position Summary: Responsible for direct referral source service delivery, providing information on programs and services to grow existing business and develop new business and coordinating referrals to ensure optimal transitions from facility to home.

Compensation based on experience $100,000 - $115,000 per year

Reports to: Regional Manager, Transitional Care & Business Development

Functions & Responsibilities:

  • Collaborates with clinical and hospice teams to promote smooth transitions from facility to home and promotes optimal communication between VNA Care and referral sources.
  • Participates in direct referral source servicing by maintaining key accounts, building new business, identifying needs, problem solving and meeting or exceeding expectations of referral sources.
  • Educates key targets on the benefits and quality of care for patients using VNA Care and elicits feedback regarding programs and services
  • Participates in developing a sales and marketing plan for home health, palliative care and hospice services with the goal of enhancing account relationships and increasing referrals.
  • Develops and implements strategies for building relationships with physicians, referral sources, managed care and assigned accounts.
  • Assists with the implementation and promotion of specialty programs designed to meet the needs of physician, referral source and managed care customers.
  • Represents VNA Care and participates in meetings, assigned presentations, educational programs and conferences, assists with coordinating with referral sources as needed.
  • Develops and maintains a work plan based on identified needs and prepares reports and maintains records in accordance with agency/department expectations.
  • Monitors external and internal environment to assess opportunities, increase market share and for competitor information.
  • Coordinates efforts with the Regional Manager and clinical colleagues to exceed service delivery and provide service excellence to external and internal customers.
  • Responds promptly to customer complaints, including documentation and processing of complaints in compliance with agency policy and participates in resolution to ensure complete service recovery.
  • Assists other liaisons with account coverage as needed.
  • Meets or exceeds referral and admission targets.
  • Upholds the compliance objectives and policies and procedures of the Foundation and Subsidiaries.
  • Performs other related duties of a similar nature and complexity as directed.

Skills and Knowledge:

Education

  • Associate’s Degree in clinical field required; Bachelor of Science Degree in clinical field preferred.
  • Licensed to practice in Massachusetts.

Experience

  • Minimum of 3 years clinical experience with 1 year in home health care
