In This Role, You Will:
  • Provide compassionate, patient-centered hospice nursing care
  • Develop and implement individualized plans of care in collaboration with patients, caregivers, providers, and the interdisciplinary team
  • Educate patients and families about hospice philosophy, services, and care expectations
  • Perform ongoing assessments and document patient progress, interventions, and outcomes accurately
  • Partner closely with physicians, healthcare professionals, hospice team members, and volunteers to coordinate care
  • Support patients and caregivers through meaningful education and emotional support
  • Participate in interdisciplinary team meetings, quality initiatives, and agency programs
  • Contribute to a culture of continuous learning and excellence in care

Requirements:
  • Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC).
  • Minimum of two years acute care nursing experience.
  • Three to four years of recent general medical/surgical or skilled/acute care nursing experience.
