At Visiting Nurse Service and Hospice of Suffolk (VNSHS), we've proudly served our community for decades with compassionate, high-quality care. As a Registered Nurse (Per Visit), you'll deliver skilled nursing services to patients in their homes while maintaining flexibility and autonomy in your work.
Your Impact:
The Registered Nurse (Per Visit) provides skilled nursing services and case management oversight, when assigned, to a diverse home care patient population. This role supports patient-centered outcomes through clinical excellence, collaboration, and timely documentation.
You Will:
Provide skilled nursing services to VNSHS patients in the home setting.
Adhere to infection control standards and all agency policies and procedures.
Utilize chronic disease management, health promotion strategies, motivational interviewing, and telehealth.
Apply the nursing process to assess physical, psychosocial, and environmental needs and evaluate patient goals.
Communicate and collaborate with home care physicians to maintain orders and report patient status.
Identify patient-centered goals and work collaboratively with the interdisciplinary team to prevent re-hospitalization.
Provide case management oversight when assigned, including visit scheduling and prioritization.
Demonstrate OASIS proficiency, including timely completion, chart accuracy, and participation in OASIS walk-throughs.
Attend weekly care management meetings and actively discuss patient caseload and progress toward goals.
Complete timely, accurate documentation using point-of-care technology.
Perform HHA and LPN supervisory visits and submit insurance authorization requests as needed.
Provide required state and federal patient notices.
Participate in weekend and holiday coverage as required.
Support the mission of VNSHS through professionalism, flexibility, and ongoing education.
What You Bring:
New York State RN license with current registration.
Ability to meet all NYS regulatory requirements for patient care.
Previous experience as a Registered Nurse.
Strong organizational, communication, and time-management skills.
Ability to work independently while collaborating effectively with a team.
