Job description

The Opportunity

Ovatient is seeking a PRN/Per Diem Virtual Care Medical Assistant to serve as an essential member of our interdisciplinary care team.

You will help with care coordination, patient education, and general patient support for a wide range of acute and chronic conditions, ensuring that patients receive timely, effective, and personalized care. In addition, you will also support the patient with technology navigation assuring our patients are prepared and comfortable to use technologies to enhance their care journey. We are seeking a motivated, experienced, and patient-centered team member who is dedicated to supporting high-quality care for our patients.

We are looking for someone with availability to work between 10am-7pm EST. This is an as-needed (PRN/per diem) position with variable hours based on operational demand. Shifts may include evenings and holidays. Hours are not guaranteed. Eventually this could turn into a full-time position as we scale.

We offer accessible, equitable and high-quality care to patients anytime and everywhere while augmenting a health system's current clinical offerings and extending its brand in market

Key Responsibilities:

  • Support the delivery of high-quality, safe, guideline-concordant, virtual care by providing and coordinate care for a wide range of low-acuity, non-emergent medical conditions through virtual consultations (video, phone, and asynchronous messaging). Including, but not limited to:
  • Patient intake and initial collection of patient health needs and goals
  • Evaluations include a determination of health status and care needs
  • Review patient medical documentation and or health referral forms, such as past medical records, test results or assignments, and incorporating this information into the patient's care plan and share it with the appropriate providers.
  • Efficiently guide the patient toward the appropriate therapies offered by Ovatient and its partners
  • Document any concerns, conflicting information, or other issues that surface during the evaluation process
  • Emphasize continuity of care, thus reducing or eliminating fragmentation, duplication, and gaps in treatment plan
  • Complete a technology check with patient who request support or have difficulty
  • Work closely with the Ovatient Providers, Behavioral Health Team and Registered Nurses
  • Collaborate effectively with our interdisciplinary team of healthcare professionals, including physicians, advanced practice providers, registered nurses, behavioral health providers, allied health professional and our operations team.
  • Actively monitor and manage referral queues, helping process referrals from primary care and specialty care providers and external sources for Behavioral Health services. Facilitate referrals to specialized services or community resources as needed.
  • Closely monitor patient messages, ensuring timely responses, addressing concerns, and providing guidance or escalating issues as necessary. Foster trust and engagement through open communication channels.
  • Onboard patients to treatment programs, including providing orientation to self-guided resources, content libraries, and secure asynchronous messaging with the care team.
  • Provide patient-centered, empathetic care that addresses individual patient needs and concerns across diverse populations, fostering strong relationships and patient satisfaction.
  • Maintain accurate, up-to-date electronic medical records, ensuring data security and confidentiality.
  • Participate in continuing education, quality improvement initiatives, and clinical team meetings.
  • Stay current with the latest advancements in virtual care, clinical best practices, and virtual care technologies.

Requirements:

  • Completed an accredited medical assistant program, nursing assistant program or an LPN with compact license
  • Possess a current national certification in medical assisting from an organization such as AAMA or NHA
  • Minimum 5 years of clinical experience in primary care or urgent care.
  • Intermediate to advanced computer skills as evidenced by ability to navigate multiple systems utilizing the same computer monitor, and quickly learn and navigate software programs and applications
  • Strong proficiency in telemedicine and virtual care technologies.
  • Experience and proficiency working with Electronic Medical Records (EMR) software
  • Exceptional communication, interpersonal, and problem-solving skills.
  • Must possess effective telephonic and virtual communication skills with a demonstrated ability to provide excellent customer service.
  • A strong commitment to patient care, quality, and clinical excellence.
  • Comfortable working independently in a fast-paced, dynamic environment.
  • Flexibility to work varying shifts, including weekends and holidays, as needed.

Preferred Qualifications

  • Have at least 2 years in a patient-centered medical home (PCMH), supporting the delivery of integrated primary care and behavioral health services
  • Experience with CMS Chronic Care Management, Remote Patient Monitoring and Remote Therapeutic guidelines, and documentation practice
  • Expertise with the Epic electronic medical record
  • Bilingual in Spanish and English
  • Previous experience in supporting care management including knowledge of complex care management and care management principles
  • State certification as a MA, NA or LPN

Remote (Work-At-Home) Requirements

  • To ensure an ability to work effectively remotely (while at home), the following is required:
  • A high-speed, reliable broadband internet connection
  • Ovatient will provide computer and telephonic equipment appropriate to meet the job requirements
  • A dedicated space isolated from interruptions to ensure you, your patients and care team have continuous privacy to accomplish daily activities and protect member confidential information (per compliance with HIPAA privacy laws)
