Key Responsibilities
  • Perform comprehensive initial and ongoing patient assessments to determine home health needs.
  • Develop, implement, and update individualized plans of care in collaboration with physicians and the care team.
  • Provide direct skilled nursing care, including medication administration, treatments, and preventive and rehabilitative interventions.
  • Identify changes in patient condition and communicate promptly with the physician to obtain updated orders.
  • Educate and counsel patients and families on care needs, disease management, and discharge planning.
  • Serve as case manager when assigned, coordinating care for an assigned caseload.
  • Complete accurate, timely documentation in the electronic medical record.
  • Participate in case collaboration, supervise home health aides (as assigned), and support care plan oversight.
  • Participate in on-call rotation as needed.

Qualifications
  • Graduate of an accredited school of nursing.
  • Current RN license in state of practice and CPR certification.
  • One (1) year of home health experience preferred.
  • Strong clinical judgment, communication, and critical thinking skills.
  • Valid driver's license, reliable transportation, and required insurance coverage.
