Job description

The Role:

Are you passionate about improving patients' lives? Join our Clinical Care team at Arine! As a Virginia licensed Clinical Pharmacist, you will utilize Arine's software platform to optimize patient outcomes, enhance patient care, and efficiently deliver our core clinical services. You will have a direct impact on the patients you serve while staying continuously up-to-date on developments in the fast-moving drug field, including emerging patterns, treatment guidelines, and best practices.

Under the supervision of a Pharmacist Lead, you will provide expert-level medication therapy consultations to patients and providers, complete medication reviews and adherence counseling, collaborate with the Engineering team and leadership to improve Arine's software platform, and help guide the Clinical Support team.

This is a patient-facing, phone-based role where you will spend a significant portion of your day engaging with patients, providers, and pharmacies. We are seeking a patient-centric, compassionate clinician who is self-motivated, receptive to coaching and feedback, and thrives on helping others navigate complex medication challenges.

Success in this role requires strong clinical judgment, attention to detail in documentation, comfort working with technology, and the ability to adapt in a fast-paced environment. You will be expected to manage your workload independently while demonstrating accountability and reliability in your work. Arine fosters a metrics and feedback-driven environment where ongoing coaching is used to support individual growth, performance improvement, and development.

What You'll Be Doing:

  • Engaging patients over the phone to deliver Arine's clinical services, including conducting Medication Therapy Management (MTM) services such as Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews (TMRs), medication adherence counseling, medication reconciliation, and clinical program education on behalf of their health plan

  • Reviewing completed medication reviews and counseling by other Virtual Pharmacist users, as assigned

  • Evaluating, assessing, and documenting patient data in Arine's software platform and other programs; preparing written reports and providing oral reports concerning patient care and team progress

  • Following up with clinics, providers, pharmacies, and patients to resolve patient issues and concerns in a timely manner, including transitions of care

  • Developing intervention strategies to optimize medication utilization, achieve quality measures, and meet other established goals

  • Managing a consistent volume of patient, provider, and pharmacy interactions while maintaining productivity and service quality

  • Meeting individual and team performance metrics related to outreach, engagement, documentation quality, and clinical outcomes

  • Adapting and rotating across clinical projects and assignments based on evolving business and client needs

  • Identifying opportunities to improve workflows and Arine's software platform by participating in meetings, collaborating cross-functionally, submitting tickets, and leveraging automation and technology to drive efficiency

  • Providing daily sign-out and maintaining clear handoff communication to ensure continuity of care

  • Coaching and supporting Clinical Care team members and orienting new Virtual Pharmacist users to workflows and the platform

Who You Are and What You Bring:

  • Doctor of Pharmacy (PharmD) from an accredited college/university

  • Virginia Licensed Pharmacist and in good standing (multiple state licenses are a plus)

  • Knowledge of CMS quality measures, Star Ratings, HEDIS metrics, and Medicare Part C and Part D medication measures

  • Knowledge of chronic disease states across multiple therapeutic areas (e.g., cardiology, pulmonology, endocrinology, behavioral health), including management of patients with polypharmacy

  • Knowledge of prescription and non-prescription drugs and drug products and their indications, efficacy, dosage, mechanisms of action, major contraindications and potential side effects

  • Ability to review and assess the appropriateness of prescriptions, considering the influence of patient age, sex, concomitant disease states, concurrent drug therapy, foods, and diagnostic procedures on drug activity and disposition

  • Ability to engage with patients, promote adherence, and apply motivational interviewing and counseling techniques

  • Excellent written and oral communication skills, with the emotional intelligence to build rapport, navigate sensitive interactions, and remain patient-focused

  • Experience working with patients of different cultural, religious, and socio-economic backgrounds

  • Demonstrated reliability, accountability, and follow-through

  • High attention to detail with a strong commitment to documentation accuracy

  • Comfort working in a metrics-driven environment where outcomes and quality are tracked

  • Ability to work independently - applying sound judgment, organizing and prioritizing competing responsibilities, and meeting deadlines with minimal oversight

  • Ability to work effectively within a team, foster effective working relationships, and build consensus

  • Openness to coaching, feedback, and continuous learning

  • Ability to adapt and thrive in a fast-paced environment, managing multiple Medicare measures and clinical priorities while navigating evolving workflows and technologies

  • Knowledge of local, state, and federal regulatory requirements related to areas of functional responsibility

  • Strong computer skills, including proficiency in Google Workspace and Mac (Apple) environment, with comfort using collaboration and contact-center tools such as Slack and Five9, navigating multiple browser-based systems simultaneously, utilizing keyboard shortcuts, maintaining a high typing speed (WPM), and performing basic technical troubleshooting to stay productive in a phone-based environment

  • Willingness and ability to obtain registered pharmacist license in other states, as needed

  • Ability to work 40 hours/week (within company operating hours of Monday-Friday, 6AM-6PM Pacific Time, with up to two rotating Saturday shifts/month)

Nice-to-Haves:

  • Experience in an ambulatory care setting, particularly in chronic disease management (e.g., diabetes, cardiovascular disease, behavioral health)

  • Inpatient, hospital, or emergency department pharmacy experience with direct patient and provider interaction

  • Experience working with geriatric populations or in long-term care settings

  • Prior experience performing MTM services with various MTM platforms

  • Experience working in a health plan, Pharmacy Benefit Manager (PBM), Accountable Care Organization (ACO), or managed care pharmacy setting

  • Experience in a high-volume healthcare or pharmacy setting (e.g., retail, community, or metrics-based call center environment)

  • Board certification in a specialty area

  • PGY1 residency in Community-based or Managed Care Pharmacy; PGY2 residency in Ambulatory Care, Geriatric Care, Emergency Medicine, or Health-System Pharmacy Administration

  • Prior experience working in a remote or virtual healthcare environment

  • Experience with other electronic medical records

  • Reliable internet capable of low latency with the ability to carry video and voice calls without breaking up

  • This role is remote, but you will be required to come to on-site meetings multiple times per year. This may be in the interview process, onboarding, and team meetings

Seasonality & Schedule:

Our Clinical Care team operates within a seasonal business cycle, and we want candidates to have a clear picture of what to expect. During peak season (May through early December), our team plays a vital role in supporting business operations. Workload and scheduling demands increase during this time, and time off may be limited. Optional overtime may also be offered to meet business needs. Each year, the Clinical Care department observes a holiday shutdown of approximately 2-3 weeks between late December and early January. Please note that while Arine remains operational, the Clinical Care department is closed during this period. Following the shutdown, the team transitions back to full capacity on a modified schedule through February, during which some shifts or hours may differ from standard operations. We value transparency and encourage candidates who thrive in dynamic, seasonal environments to apply.
