Human Touch Home Health is seeking an organized, detail-driven, and customer-focused Intake Specialist to join our Falls Church, Virginia office. This is a full-time, in-office position for someone who understands how important accuracy, communication, and follow-through are in home health.

Every new patient starts somewhere.

A hospital sends a referral.

A physician needs services arranged.

A family is trying to figure out what happens next.

Intake Is Where Great Home Health Begins

This isn't just a data-entry position.

As an Intake Specialist, you're helping determine whether we can safely and appropriately accept a patient for care-and then making sure everything needed to begin that care gets where it needs to go.

You'll communicate with referral sources, physicians, hospitals, patients, families, clinicians, and internal departments to help move referrals from "received" to "ready for care."

Some days will move fast.

Referrals will come in.

Phones will ring.

Documents will be missing.

Insurance will need to be verified.

Someone will need an answer.

That's why we're looking for someone who doesn't just see a problem and stop.

We're looking for someone who asks:

"What do we need to do to get this patient taken care of?"

What You'll Do
  • Receive and process incoming home health referrals
  • Review referral documentation for completeness and accuracy
  • Enter patient and referral information accurately into company systems
  • Verify patient demographics and insurance information
  • Perform insurance eligibility and benefits verification
  • Communicate with hospitals, physicians, rehabilitation facilities, case managers, and other referral sources
  • Obtain missing clinical documentation, physician orders, face-to-face documentation, and other required information
  • Coordinate with clinical leadership to determine whether referrals meet admission criteria
  • Communicate accepted referrals and Start of Care needs to appropriate clinical and scheduling teams
  • Maintain accurate referral status and follow up on pending referrals
  • Communicate professionally with patients and families regarding the intake process
  • Ensure documentation is completed according to agency, payer, and regulatory requirements
  • Maintain patient confidentiality and HIPAA compliance
  • Assist with tracking referral volume, admissions, pending referrals, and referral outcomes
  • Work closely with clinical, scheduling, billing, recruiting, and administrative teams to support smooth patient admissions

What You'll Need
  • High School Diploma or GED Required
  • Previous Intake, Healthcare Administration, Medical Office, or Home Health Experience Preferred
  • Home Health Intake Experience Strongly Preferred
  • Knowledge of Medicare, Medicaid, and Commercial Insurance Requirements Preferred
  • Experience With Insurance Eligibility Verification Preferred
  • Familiarity With Home Health Referral and Admission Processes is a Plus
  • WellSky Experience is a Plus
  • Proficiency in Microsoft Word, Excel, Outlook, and General Computer Systems
  • Strong Written and Verbal Communication Skills
  • Exceptional Attention to Detail
  • Strong Organizational and Time-Management Skills
  • Ability to Prioritize Multiple Referrals and Deadlines
