Dietitian - Healthy Results Wellness Program

This is a hybrid position that will work 3 days a week 8am-5pm, 1 day a week 10:30am-7pm, and 1 day a week 11:30am-8pm. Provides telephonic nutrition counseling and education related to applicable disease states for IU Health team members enrolled in Healthy Results, IU Health's team member wellness program, through self-referral, health coach referral, and/or risk stratification processes.

Job duties include:

  • Utilize adult education techniques to educate patients about disease risk factors and prevention, disease state, role of medications, and medical nutrition counseling related to management of chronic diseases including diabetes.
  • Assess, implement, evaluate, and conduct follow-up on all participants enrolled in dietitian program. Identify gaps in care and communicate/navigate to appropriate referrals.
  • Conduct proactive outreach and intervention with risk stratified individuals.
  • Act as a resource person to the patient and other health care professionals within Healthy Results.

Success is accomplished by conducting telephonic/virtual educational sessions via one-on-one, group setting webinars, and workshops; following National guidelines on standards of care; participating in cross-training with the health coaching team; and updating patient education materials and protocols.

  • Master's Degree is required or Bachelor's Degree prior to 2024.
  • Requires 3-5 years of relevant experience.
  • Registration through the Commission on Dietetic Registration must be obtained within 6 months of start date.
  • Licensure through the state of Indiana must be obtained within 1 year of start date.
  • Any clinical area billing for service may require CDR approval and Indiana state RD license prior to start date.
  • Requires the ability to develop nutritional care plans and provide nutritional education.
  • Requires the ability to demonstrate competency in nutrition focused physical exam and malnutrition documentation.
  • Certified Diabetes Educator preferred
  • Minimal Travel for in-state presentations
  • Wellness program experience preferred
