Job description

SUMMARY
The role of the Health Care Navigator is to drive post-acute and long-stay census results for the Toledo Ohio Otterbein ministries. The position will work on a complementary basis with the Senior Lifestyle Communities; Sunset House, Sunset Village, Pemberville & Marblehead- along with Otterbein Small House Neighborhoods; Monclova, & Perrysburg. With a specific focus on: Otterbein Perrysburg, Otterbein Monclova, Otterbein Sunset House, Otterbein Sunset Village, and Otterbein Pemberville. With a targeted emphasis on the Toledo Ohio Hospital Systems (key accounts- Promedica, Mercy, UTMC) and other secondary/tertiary healthcare systems; doctoral practices, Chambers of Commerce, free standing IL/AL, social/business groups, senior centers and churches in the identified geography. Assist with developing and executing strategies for social media/digital marketing campaigns.
Responsibilities:
  • Establish relationships with hospital key accounts (administrators, doctors, social workers, and discharge nurses) to drive referrals for patients needing long-term nursing and/or post-acute (rehabilitative) care.
  • Ensure financial viability of the local ministries by developing and maintaining a relationship that yields an increased share of patient referrals from the targeted hospitals.
  • Monitor and maintain targeted payer mix to meet financial goals and assure financial stability for the ministries supported.
  • Develop and execute quarterly marketing plans for Toledo, Ohio and surrounding areas.
  • Assess potential referrals or potential admissions, and work to admit the patient to the optimal Otterbein location, utilizing a One Otterbein approach.
  • Distribute brochures, flyers and other promotional items to referral sources in the targeted accounts.
  • Work closely and collaboratively with hospital and Otterbein admissions/discharge marketing team members to reassure easy transition of patient from hospital to Otterbein.
  • Coordinate marketing efforts to meet census goals and to keep the Otterbein location staff advised of anticipated referrals, tours and/or admissions.
  • Identify opportunities and execute those selected opportunities to make working with Otterbein easier and more efficient for acute care and doctoral practice personnel.
  • Create digital marketing strategies applicable to the geography supported by this position.

  • Work closely with centralized intake and Lifestyle Community admissions staff to ensure relevant data and messaging to referral partners is timely and accurate.
  • Work strategically with Otterbein locations and Promedica/Mercy ACO to gain market share and meet metrics required for affiliation status.
  • Collecting and evaluating data across all ministries supported for tracking and trending of patterns.
  • Identify and pursue new opportunities for business development within both the acute care setting along with the community setting.
  • Performs any other duties as assigned.

Skills Required:
  • Recognize the autonomy and dignity of all patients/residents.
  • Communicate a sense of caring, concern, and dignity for patients/residents.
  • Understand how to place decision-making in the hands of the patients/residents when appropriate.
  • Make prompt and accurate judgments with regard to patients/residents care and emergencies.
  • Ability to work with an inter-departmental approach to census growth and development.
  • Work and communicate effectively as part of the self-directed marketing work team.
  • Establish excellent relationships and ongoing communications with hospital personnel in referral marketing, management and decision-making roles.

Qualifications:
  • 3-5 years' experience in Sales/Marketing/Clinical liaison for Skilled Nursing Provider
  • Understanding/Proficient with the skilled nursing admission process
  • Relationships in the Toledo hospitals (preferred)
  • BA/BS preferred; plus three to five years of sales/marketing experience with a proven track record in referral marketing, healthcare admissions in a senior care or nursing home setting.
  • Knowledge of data management (customer relationship management) programs preferred.
  • LPN, RN, or social worker with experience as a clinical liaison (preferred).
  • Excellent organizational/planning/time management skills.
  • Experience in supporting multiple locations/teams/key accounts
  • Outstanding verbal and written communication skills to connect with referral partners and patients/residents.
  • Virtual/Digital marketing experience preferred
  • Reliable transportation required
