Job description

Hospice Registered Nurse

Park Nicollet is looking to hire a Registered Nurse to join our Hospice team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.

Position Summary:

The Outpatient Hospice Case Manager registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice.

The nurse is responsible to improve or maintain patient's health status, provide terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting.

The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery.

This position will support the West Team (ex: Lake Minnetonka area)

Work Schedule:

4 days a week 8a-4:30p and every 4th weekend

Required Qualifications:

  • Education, Experience or Equivalent Combination:
    • Baccalaureate degree in Nursing required.
    • Work experience as a registered nurse for at least two years
  • Licensure/ Registration/ Certification:
    • Current license to practice as a professional nurse in Minnesota.
  • Knowledge, Skills, and Abilities:
    • Must be able to function independently with minimal supervision.
    • High level of sensitivity, assessment skills, technical-skills, and communications skills.
    • Demonstrates initiative, judgment, critical thinking, and the ability to problem solve.
    • Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.

Preferred Qualifications:

  • Education, Experience or Equivalent Combination:
    • Experience in hospice desirable
