) Imperial a Villa Center is seeking a Patient Care Coordinator to join their team! The Patient Care Coordinator supports residents in their transition to and throughout their stay within the facility by assisting with daily needs, activities, and comfort measures. This position works collaboratively with Nursing, Admissions, and Interdisciplinary Team members to ensure a welcoming, person-centered experience. The Patient Care Coordinator assists with admission activities, including welcoming new residents, orienting them to their new home, and facilitating introductions to their care team to promote comfort, dignity, and continuity of care.

Patient Care Coordinator Duties and Responsibilities:
  • Greets and welcomes new residents upon admission and assists with orientation to the facility, including touring the resident through their new home and reviewing available services and amenities.
  • Assists residents with daily non-clinical needs to promote comfort, safety, and overall well-being.
  • Supports residents with daily activities and routines in accordance with individualized care plans and personal preferences.
  • Collaborates with Nursing and the Interdisciplinary Team to ensure new residents are introduced to their care team and that immediate needs are communicated and addressed.
  • Assists with room readiness to ensure the resident's environment is clean, organized, personalized, and welcoming upon arrival.
  • Communicates resident concerns, preferences, and feedback to the appropriate department to ensure timely follow-up and resolution.
  • Maintains positive rapport with residents and families, promoting a supportive and person-centered environment.
  • Assists with admission-related processes and documentation as directed to ensure a smooth transition into the facility.
  • Participates in daily team meetings or rounds as requested to provide updates on new admissions and resident needs.
  • Supports resident engagement and encourages participation in facility programs and activities as appropriate.
  • Perform all other duties, as assigned.

Patient Care Coordinator Skills and Abilities:
  • Ability to relate positively, effectively, and appropriately and independently with patient and families.
  • Experience working on Care Transition Unit would be preferred.
  • Strong verbal and written communication skills in English to perform the tasks required.
  • Sufficient computer skills required to perform duties.

Patient Care Coordinator Education and Experience:
  • High school diploma or equivalent.
  • At least one year of prior experience in a relevant role, preferred.
  • At least one year of experience working in a healthcare setting.

Physical Requirements:
  • Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required.
  • Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.
  • Meet all health requirements as needed.
  • Dialogue concerning ADA reasonable accommodations are always available upon request - i.e., the interactive process.
