About Carease Health

Carease Health is a nationwide preventative healthcare company focused on reducing hospitalizations and improving outcomes for high-risk patients in skilled nursing facilities. We deliver Advanced Primary Care Management (APCM), Remote Patient Monitoring (RPM), and Collaborative Care Management (CoCM) services across hundreds of SNF and long-term care facilities. We are an established, privately held company operating at scale.

The Role

We're looking for a VP Platform & Partnerships who has spent the last decade inside the LTPAC software ecosystem. You know the major platforms, the specialty vendors, and the integration economics of this space because you've lived in it.

This role owns Carease's technology partnership strategy — evaluating vendors, negotiating agreements, and directing how the pieces fit together. You'll report to the CEO and partner closely with our VP of Clinical & Informatics and Systems Architect.

What You Will Do

  • Own vendor strategy and partnership decisions across Carease's technology stack

  • Evaluate, negotiate, and structure licensing and integration agreements

  • Make build-vs-license-vs-buy recommendations

  • Maintain working relationships with major LTPAC platforms

  • Direct the Systems Architect on integration priorities

What We Are Looking For

  • 5–10 years inside the LTPAC software space, ideally at a major platform or LTPAC services company

  • Track record of vendor evaluation and partnership negotiation

  • Working knowledge of the SNF/LTPAC software ecosystem

  • Familiarity with APCM, RPM, CCM, and CoCM technology requirements

  • Strong communication with clinical, operational, and executive stakeholders

Strongly Preferred

  • Prior role at a leading LTPAC platform

  • Background in healthcare technology partnerships or business development

Location & Compensation

Hybrid for the right candidate, with periodic presence in Lakewood, NJ. Base salary range: $200,000–$280,000, commensurate with experience. Comprehensive benefits package including medical, dental, and vision coverage, paid time off, and 401(k).
