Position Summary The MDS Coordinator (RN) is responsible for coordinating and completing comprehensive resident assessments in accordance with federal and state regulations, facility policies, and quality standards. This role plays a key part in ensuring accurate clinical documentation, care planning, reimbursement optimization, and regulatory compliance while supporting the delivery of exceptional resident care. Key Responsibilities Conduct and coordinate the completion of resident assessments (MDS) in accordance with federal and state requirements.

Ensure the timely and accurate completion of MDS, Care Area Assessments (CAAs), and care plans. Collaborate with interdisciplinary team members to develop and maintain individualized resident care plans. Monitor assessment schedules and ensure compliance with Medicare, Medicaid, and regulatory guidelines.

Review clinical documentation to support accurate coding and reimbursement. Assist with surveys, audits, and quality improvement initiatives. Serve as a clinical resource for nursing staff regarding assessment and documentation requirements.

Maintain current knowledge of MDS regulations and reimbursement processes. Qualifications: Required Nursing degree from an accredited college or university. Current, active, and unencumbered Registered Nurse (RN) license in the state of practice.

Minimum of one (1) year of experience in a hospital, long-term care facility, or related healthcare setting. Experience completing and coordinating MDS assessments in a long-term care or skilled nursing facility. Strong organizational, analytical, and communication skills.

Knowledge of federal and state regulations related to resident assessments and care planning. Preferred RAC-CT certification or advanced MDS training. Experience with Medicare, Medicaid, and skilled nursing reimbursement processes.

Previous experience in long-term care clinical leadership or care coordination roles.
