Job description

Bilingual Medical Case Manager

Location: Greenville, SC, 29601

Overall Purpose

The purpose of this position is to provide Spanish-only speaking people affected by HIV in Anderson, Greenville, Oconee, and Pickens Counties access to a range of client-centered supportive services, and to assist clients and their families with personal and environmental difficulties that interfere with obtaining maximum benefits from care.

Essential Functions
  • Be present for all work shifts.
  • Arrive at least 10 minutes before workshift begins.
  • Remain on the job site for his/her entire shift.
  • Speak fluently in both Spanish and English.
  • Provide all services in a culturally appropriate manner.
  • Provide accurate, impartial, and culturally appropriate interpretation in the client’s preferred language to facilitate communication between clients and AID Upstate staff and to help bridge cultural and linguistic barriers.
  • Assist AID Upstate medical staff in communicating medical instructions, treatment recommendations, and health information to Spanish-speaking clients through in-person, telephone, or conference-call interpretation as needed.
  • When available, provide interpretation and translation assistance to AID Upstate Prevention Services staff and other internal departments as needed.
  • When requested by clients and approved by management, attend external medical, behavioral health, social service, or case management appointments to provide support and facilitate access to care.
  • Maintain client confidentiality and follow HIPAA guidelines at all times.
  • Work in close collaboration with physicians, nurses, nurse educators, therapists and other health care personnel in client evaluation and treatment to further their understanding of significant social and emotional factors underlying client’s health problem.
  • Participate in planning for improving health services by interpreting social factors pertinent to development of client.
  • Prepare client histories, action plans, and service and medical reports.
  • Assist clients in obtaining and maintaining enrollment in benefit programs geared towards access to care and treatment.
  • Utilize community resources to assist client to resume life in community or to learn to live within limits of disability.
  • Help client (and family) through individual or group conferences to understand, accept and follow medical recommendations.
  • Provide services planned to restore client to optimum adherence to treatment, within client’s capacity.
  • Document all client-related activities into Provide Enterprise (PE) according to agency and SC DHEC policy.
  • Communicate with clients, co-workers, other healthcare professionals and vendors via telephone, email, fax and pen and paper.
  • Manage a caseload of 100 or more individuals.

Responsibilities

The employee is responsible for assisting clients (and their families) with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care, and doing so with empathy.

The employee must complete all necessary and required trainings in a timely manner and with a passing score. The employee must stay abreast of services available to clients, as well as the requirements of attaining/retaining those services. The employee is responsible for adhering to a strict confidentiality policy, both at work and away from work.

Note: AID Upstate’s confidentiality agreement extends beyond the employee’s employment period.

This employee serves as an internal language resource for AID Upstate and may provide interpretation and translation services in support of company programs and client care activities. Interpretation services are intended for AID Upstate operations and clients. Employees in this position are not required to provide interpretation services on behalf of external agencies, providers, or organizations outside AID Upstate, even when AID Upstate is the referring agency.

QUALIFICATIONS
Education/Experience
A Bachelor’s degree from an accredited 4-year college or university in a behavioral science and two years related experience and/or training (or, an equivalent combination of education and experience) is required.
Computer Skills
To perform this job successfully, and individual must have knowledge of basic office equipment including multi-line telephone, facsimile (fax) machines, copiers, computers, scanners and printers. Experience with Microsoft Word, Outlook Email, basic computer database software and Internet software required.
