• Conduct thorough psychosocial assessments to understand clients' mental health concerns and resources, ensuring effective treatment planning.
• Deliver individual, family, and group therapy sessions utilizing evidence-based modalities such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed practices.
• Develop clear, individualized treatment plans that set goals and outline specific interventions tailored to each client’s unique circumstances.
• Collaborate seamlessly with a multidisciplinary treatment team including psychiatrists, psychologists, and allied health professionals to provide coordinated care.
• Implement crisis intervention strategies, carry out risk assessments, and enact safety plans to support clients in crisis situations.
• Maintain precise and timely documentation, ensuring compliance with ethical guidelines and legal standards while adhering to best practices in mental health reporting.
• Facilitate engaging group therapy sessions, psychoeducational classes, and support groups that empower clients and promote community healing.

• A Master's degree in Social Work from an accredited program is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Minimum of two years of professional experience in a mental health or outpatient setting.

• Exceptional interpersonal, communication, and assessment skills are imperative for effective client engagement.

• Experience working in a hospice or palliative care environment, with an understanding of the unique emotional needs of clients facing life-limiting illnesses.
• Familiarity with telehealth practices and electronic health record systems to facilitate efficient service delivery.
• Proven ability to provide culturally competent care for diverse populations.
