Housing Case Manager (TH-RRH)

Location: West Palm Beach, FL

Schedule: Full-time (40 hours/week) | Weekdays with occasional evenings/weekends

Position Summary:

Vita Nova is seeking a compassionate and resourceful Housing Case Manager to support young adults experiencing homelessness through its dual-component Transitional Housing to Rapid Re-Housing (TH-RRH) program. This position provides intensive, housing-focused case management, life skills development, employment and education support, and connection to community resources. Working both in an office and throughout the community, the Housing Case Manager partners with youth to achieve housing stability, improve well-being, and build pathways toward self-sufficiency.

Key Responsibilities:

โ€ข Provide intensive case management and develop individualized service plans focused on housing stability and goal achievement

โ€ข Assist youth with housing searches, lease agreements, landlord relationships, and tenant education

โ€ข Facilitate individual and group life skills training, including budgeting, employment readiness, and independent living skills

โ€ข Connect youth to community resources, including healthcare, mental health services, education, employment, and supportive adult mentors

โ€ข Maintain accurate client documentation, case notes, and program data within HMIS and agency systems

โ€ข Conduct assessments, monitor participant progress, and support crisis intervention and safety planning

โ€ข Provide transportation and community-based services as needed

โ€ข Participate in community partnerships and outreach efforts supporting homeless and disconnected youth

Qualifications:

โ€ข Bachelor's degree in Social Work, Human Services, or related field required

โ€ข Experience working with young adults, or individuals experiencing homelessness preferred

โ€ข Knowledge of Trauma-Informed Care and Positive Youth Development practices

โ€ข Ability to identify community resources and assist youth with housing, employment, and life skills development

โ€ข Strong crisis intervention, case management, documentation, and organizational skills

โ€ข Proficiency in Microsoft Office Suite and general computer literacy

โ€ข Excellent written and verbal communication skills

โ€ขย ย Ability to work independently and collaboratively in fast-paced community setting

โ€ขย ย Working knowledge of Microsoft Office and ability to document services in HMIS

โ€ขย ย Reliable transportation with a valid FL driver's license and ability to join company insurance policy

โ€ขย  Ability to pass drug screen and a Level 2 background check through DCF Clearinghouse: https://info.flclearinghouse.com (Existing DCF Exemption Waivers will be considered)
