Job description

Case Manager (Interim Housing) - Swing Shift
Schedule: 3:00 PM - 11:30 PM
About the Role
As part of the Interim Housing team, the Case Manager plays a critical role in helping individuals break the cycle of homelessness. This position supports participants across all stages of homelessness, including street outreach, interim housing, transitional housing, and permanent housing placements.
Responsibilities
  • Conduct comprehensive intakes and individualized needs assessments for all participants
  • Develop and maintain Individualized Service Plans (ISP) addressing housing goals, income growth, and personal needs including medical, mental health, substance use, vocational, financial, and social support services
  • Provide referrals, linkages, and ongoing support to community resources aligned with participant goals
  • Maintain accurate, timely case notes and documentation in accordance with program and compliance standards
Preferred Qualifications
  • 2+ years of experience in case management, healthcare, homeless services, or working with vulnerable populations
  • Experience working in interim housing or shelter settings
  • Familiarity with HMIS and Coordinated Entry System (CES) preferred
  • Comfortable supporting individuals with mental health, substance use, and complex needs
  • Willingness to work within a harm reduction and Housing First model

Pay Details: $23.00 to $26.00 per hour
