Job description

Tranquility Hospice and Palliative Care is seeking compassionate team players to join our hospice nursing team. We have full and part-time openings for Registered Nurse Case Managers and Admission Nurses. Weeknight and weekend on-call rotation. IN licensure required. Hospice experienced preferred.

Summary of essential job functions:

  • Serves as the primary care nurse for all assigned cases and provides skilled nursing services.

  • Observe, evaluate and report to the attending physician and interdisciplinary team patient's physical and emotional conditions and reactions to drugs or treatments.

  • Assists in developing a pain management and symptom management plan to keep the patient as comfortable as possible.

  • Reassess pain and notifies the primary care physician when pain is not manageable on current medication.

  • Develops a patient care plan utilizing the written physician's orders, communicates and interprets the goals of care to the patient and family.

  • Completes written documentation for the medical record, assist the family to accept the responsibilities for providing care in the home and provides emotional support for these activities.

  • Supervises hospice aides.

Requirements:

  • Must be an active Registered Nurse (RN) for the State of Indiana.

  • Minimum of 1 years' experience, preferably in the area of Hospice.

  • Prolonged or considerable walking or standing.

  • Able to lift, position, or transfer patients.

  • Able to lift supplies and equipment.

  • Considerable reaching, stooping, bending, kneeling, or crouching.

  • Visual acuity and hearing to perform required nursing skills.
