Position Summary
The Community Liaison serves as the primary business development representative for Tranquility Hospice. The primary responsibility of this position is to establish, develop, and maintain professional relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, home health agencies, case managers, social workers, discharge planners, and other healthcare professionals to educate referral sources about hospice services and promote appropriate referrals.
The Community Liaison customarily and regularly performs the majority of work duties away from the employer's office through face-to-face meetings, presentations, networking, and community outreach activities. This position focuses exclusively on business development and professional relationship management and does not participate in clinical decision-making or determine patient eligibility for hospice services.
Essential Duties and Responsibilities
- Develop and execute strategic business development plans to increase awareness of Tranquility Hospice within the assigned service area.
- Identify, prioritize, and cultivate relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, rehabilitation centers, home health agencies, dialysis centers, oncology practices, and other healthcare referral sources.
- Conduct regular in-person visits with referral partners to educate healthcare professionals regarding hospice eligibility, available services, and the benefits of hospice care.
- Represent Tranquility Hospice at networking events, health fairs, professional meetings, educational programs, and community outreach activities.
- Analyze the assigned market to identify growth opportunities, referral trends, competitive activity, and target accounts.
- Develop and implement annual and quarterly marketing objectives and territory growth strategies.
- Maintain ongoing relationships with referral sources through regular face-to-face visits, follow-up communications, and educational presentations.
- Coordinate educational in-service presentations for healthcare providers and community organizations.
- Promote Tranquility Hospice's mission, values, services, and commitment to quality patient care.
- Maintain accurate documentation of sales activities, referral source contacts, outreach efforts, and account interactions within the company's CRM or other approved systems.
- Monitor marketing performance and recommend improvements to business development strategies.
- Assist with physician relationship management by delivering requested documentation, obtaining physician signatures when appropriate, coordinating communications, and supporting professional outreach activities.
- Attend interdisciplinary team (IDG) meetings as appropriate to remain informed regarding hospice services and facilitate community education efforts. Participation in IDG meetings does not include making or influencing clinical eligibility or admission decisions.
- Maintain knowledge of Medicare, Medicaid, hospice regulations, and industry developments relevant to community education and business development.
- Perform other related business development duties as assigned.
Clinical Independence
The Community Liaison shall not:
- Determine whether a patient qualifies for hospice services.
- Participate in clinical evaluations or eligibility determinations.
- Influence or direct clinical admission decisions.
- Recommend admission of any patient based on financial or incentive considerations.
All patient eligibility determinations and admission decisions are made independently by licensed clinical personnel based solely upon medical necessity and applicable Medicare, Medicaid, and payer requirements.
Qualifications
Required
- Minimum one (1) year of experience in healthcare marketing, business development, nursing, social work, physician relations, or a related healthcare field.
- Excellent interpersonal, communication, presentation, and relationship-building skills.
- Strong organizational and time-management abilities.
- Ability to work independently and effectively manage a geographic territory.
- Proficiency using Microsoft Office and customer relationship management (CRM) software.
- Valid driver's license, reliable transportation, and automobile insurance.
- Ability to travel throughout the assigned service area on a daily basis.
Preferred
- Experience in hospice, home health, palliative care, long-term care, physician outreach, or healthcare sales.
- Existing professional relationships within the local healthcare community.
Physical Requirements
- Frequent local travel throughout the assigned service area.
- Frequent driving between healthcare facilities and physician offices.
- Ability to sit, stand, walk, and carry marketing materials as needed.
- Ability to occasionally lift up to 25 pounds.
Work Environment
This position is primarily field-based. The Community Liaison customarily and regularly performs work away from the employer's office while visiting referral sources, healthcare facilities, and community organizations. Administrative tasks, reporting, and planning activities may be completed remotely or from the office as directed.
Compensation
Compensation may consist of a base salary or hourly wage, together with eligibility for incentive compensation under the company's Business Development Incentive Plan. Any incentive compensation is based on legitimate business development objectives and territory performance and is subject to applicable company policies and healthcare compliance requirements.
