• Conduct thorough assessments, diagnostic evaluations, and ongoing assessments of patients using evidence-based practices tailored to individual needs.
• Design and implement individualized care plans that incorporate cultural diversity while addressing the patient's mental health diagnosis and their specific set of needs.
• Maintain accurate and timely documentation of treatment records and reports, leveraging current technology and adhering to regulatory guidelines.
• Collaborate with patients, families, and treatment providers to establish comprehensive treatment plans, ensuring continuous evaluation of progress and feedback on therapeutic interventions.
• Coordinate care and referrals, acting as a vital link between clients and other mental health professionals and community resources.
• Provide education, mentoring, and supervision to students, interns, and volunteers, while engaging in clinical collaboration and refining best practices.

This role primarily focuses on patients who are navigating the complexities surrounding cancer diagnoses. As a member of a supportive team, you will engage with individuals and families who require psychological support, coping strategies, and therapeutic interventions that foster resilience and recovery. This population often faces unique technological, emotional, and logistical challenges, requiring tailored communication and a deep understanding of their experiences.

• A Master’s degree in Social Work (MSW) from an accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Texas.
• Demonstrated ability to work effectively with diverse populations and varying age groups.

• Previous experience in a clinical setting, particularly within school, outpatient, or community health environments.
• Familiarity with trauma-informed care practices and evidence-based therapeutic modalities.
• Experience managing caseloads and coordinating care across multidisciplinary teams.

Sun Rise Horizon Counseling Services offers an attractive salary range of $90,000 to $105,000 annually, complemented by a generous benefits package. Our offerings include:
