Description
StarCare Specialty Health System (StarCare) is in search of a well-organized and dynamic professional to join the Local Intellectual and Developmental Disability Authority (LIDDA) as the Continuity of Care (CoC) Service Coordinator. This position completes enrollment processes for Medicaid Waiver Programs, provides Permanency Planning for individuals under the age of 22 residing in Intermediate Care Facilities for Persons with Intellectual Disability or Related Conditions, and serves as the Designated Local Intellectual and Developmental Disability Authority and liaison for people who reside at the Lubbock State Supported Living Center (SSLC).
Responsibilities:
  • Monitors the status of individuals residing in State Supported Living Centers who have transitioned into the community within the past year to ensure needs of the individuals are met.
  • Collaborates and coordinates with Local Intellectual and Developmental Disability Authorities, State Supported Living Centers, State Hospitals, Nursing Facilities, and community provider staff specific to community living transitions, to ensure continuity of services.
  • Completes and submits accurate and timely documentation, including but not limited to:

o Service delivery notes, daily timesheet entries, mileage logs, Intermediate Care Facility (ICF) admissions, Intellectual Disability or Related Conditions (ID/RC) forms, Waiver Enrollments, and Permanency Plans.
  • Adheres to all attendance expectations, following established guidelines to ensure accountability of hours worked each week, maximum productivity, and time management.
  • Performs other duties as assigned.

Schedule: This is a full-time/non-exempt position scheduled Monday - Friday, 8am - 5pm, with flexibility to meet the needs of the department.

Qualifications:
  • Bachelor's degree from an accredited four (4) year college or university is required. Degrees in a human services field such as social work, sociology, psychology, family studies, etc. is preferred.
  • One (1) year of full-time experience working in the field of intellectual & developmental disabilities, preferred.
  • Experience with SmartCare, TMHP, CSIL, SASO, and CARE, preferred.

Skills & Abilities:
  • Knowledge of human service delivery systems.
  • Knowledge of state supported living centers (SSLC).
  • Ability to communicate effectively, both orally and in writing.
  • Ability to organize, prioritize a variety of assignments and manage time effectively.
  • Ability to maintain effective and positive working relationships.
  • Knowledge and skill in operating standard office equipment (personal computer, phone, fax, printer, copier, etc.) and Microsoft Office products.

Additional Requirements:
  • Must pass a pre-hire drug screen and criminal background check.
  • Must have a Texas driver's license, Texas automobile liability insurance coverage, and be insurable under the Agency's insurance.
