Description
Performs minimally complex social service work. Reviews Texas Medicaid & Healthcare Partnership (TMHP) internet portal for new and pending assessments. Responsible for conducting screening and evaluation of person(s) seeking admission into a Medicaid-certified nursing facility. Conducts face-to-face evaluation of said individuals to assess need for specialized services, alternate placement options for the individual, and the need for care in a nursing facility or other setting, such as an intermediate care facility for individuals with an intellectual disability or related condition(s). Works under moderate supervision with latitude for the use of initiative and independent judgment.
Examples of work performed
- Daily review of TMHP internet portal.
- Contacts Nursing Facilities to schedule assessment appointments.
- Assesses client's needs and authorizes services to meet the identified needs.
- Coordinates service delivery.
- Conduct client Intake.
Requirements
Bachelor's degree from an accredited college or University with a major in social work, psychology, or a field to human behavior. One (1) year experience in working with Individual with Intellectual Disabilities required.
Must become certified to complete the State approved Uniform Assessment (Texas Resilience and Recovery) for Adults or Child/Youth within 90 days of hire and maintain that certification without lapse.
