In this full-time role, you will be responsible for providing exceptional patient-centered physical therapy services to individuals in their own homes. • Conduct comprehensive patient assessments to establish individual needs, limitations, and goals for physical therapy.
• Develop and implement personalized care plans in collaboration with physicians and auxiliary health teams, ensuring a holistic approach to recovery.
• Deliver therapeutic interventions using a variety of techniques to enhance functional mobility, strength, and overall patient well-being.
• Continuously monitor patient progress and adapt treatment plans as necessary, documenting all findings in compliance with professional standards.
• Educate patients, family members, and caregivers on exercises, therapeutic techniques, and the use of any necessary adaptive equipment.
• Prioritize patient caseload and manage scheduling to ensure efficient service delivery while maintaining quality of care.
• Provide guidance and support to patients as they transition through various stages of recovery, ensuring they feel empowered and informed.

As a Physical Therapist with Vitalize Physical Therapy, you will primarily work with adult and pediatric patients recovering from surgeries, injuries, or chronic conditions. Your ability to empathize with patients and tailor your approach to meet their individual needs will be paramount in fostering positive outcomes. You will encounter diverse cases including orthopedic rehabilitation, post-operative care, and chronic pain management, all of which require a thoughtful and adaptive treatment style.

• A valid and current Physical Therapy license in California.
• A Doctor of Physical Therapy (DPT) degree from an accredited institution.
• Basic Cardiac Life Support (BCLS) certification or ability to obtain within 30 days of employment.
• Strong communication skills and the ability to build rapport with patients and their families.
• Dedication to providing compassionate care and maintaining a patient-focused approach.

• Experience in a home care or outpatient setting is preferred but not required.
• Familiarity with rehabilitation technologies and adaptive equipment will be advantageous.
• Previous experience in developing and implementing tailored rehabilitation programs.
• Knowledge of current best practices and evidence-based treatment modalities.
