Full Circle Therapy and Wellness, a small, privately owned outpatient physical therapy clinic in De Pere, WI is seeking a compassionate, motivated and skilled physical therapist to join our team, full-time, part-time or PRN. If you are passionate about helping people regain mobility, reduce pain, and improve their quality of life, we would love to hear from you. We are committed to providing exceptional patient-centered care in a positive, collaborative environment where both our patients and our team members thrive.

Responsibilities

  • Perform comprehensive patient evaluations to assess movement abilities and limitations.
  • Develop personalized treatment plans to meet individual's goals.
  • Provide evidence-based physical therapy interventions in one-to-one, 45-min sessions.
  • Educate patients on treatment, exercises, self-care strategies, and long-term wellness.
  • Work with our PTA's to collaborate on treatment and patient care.
  • Monitor patient progress and modify treatment as needed.
  • Accurately and timely complete all patient evaluation, treatment and progress notes within PROMT EMR and maintain compliance.
  • Collaborate with physicians and other healthcare professionals to provide exceptional coordinated patient care.

Qualifications

  • Doctor of Physical Therapy (DPT) degree from an accredited program.
  • Current Wisconsin Physical Therapist license.
  • Excellent communication and interpersonal skills.
  • Proven track record for patient progress.
  • Strong clinical reasoning and problem-solving abilities.
  • Dry needling certification and experience highly desirable.
