Job description

Job Summary

The Manager Social Work provides oversight and planning for the operations of the inpatient and outpatient Social Work programs.  Oversees the assessment of patient needs, treatment plan development and the delivery of therapeutic interventions. Collaborates with Case Managers, Transitional Care Coordinator and Utilization Review nurses.  Provides education and support as a resource to supervisors and staff.  Works under the direction from the Director of Case Management.

Core Job Responsibilities
  • Accountable for ongoing and daily operations and routine decision making across all social work operations.  
  • Ensures that the Social Work, Case Management, Discharge Planning and Utilization Review teams work effectively and efficiently with the interdisciplinary health care team Ensures staffing for these areas meets patient needs.
  • Ensures proper supervision of assigned Mental Health Counselors, LMSWs, MSWs, students, residents, and unlicensed staff within scope of license.  
  • Collaborates with Behavioral Health providers and ancillary team.
  • Responsible for the delivery of social work services including assessments, individualized treatment plans, therapeutic interventions/therapy, care coordination, crisis management, chronic condition management, referral to community resources, education, etc.
  • Monitors complex cases and assists to remove barriers to care.
  • Identifies and monitors over utilization of services and reports trends.
  • Analyzes chart audits to ensure that documentation meets all regulatory requirements, patient care needs and trends.
  • Ensures that the Social Work teams are adhering to Conditions of Participation, OMH and all regulatory requirements that impact patient rights.
  • Leads quality improvement initiatives including the review and analysis of social work data to evaluate both the inpatient and outpatient social work programs.
  • Supports grant applications, implementation and related information gathering.
  • Ensures all staff provides service excellence to all patients and their families/visitors that meets or exceeds patient expectations through offering empathetic and compassionate patient-centered care and ensures resolution of complaints.
  • With the Director and Human Resources guidance, provides input to or makes recommendations on employee hires, performance reviews, transfers, promotions, disciplinary actions, terminations and similar actions. Resolves grievances and other staff problems in a fair, timely and consistent manner also in partnership with HR.
  • Oversees orientation and training for new hires.  Ensures all staff have the resources, tools, information and training necessary to fulfill their duties.  Promotes and role models teamwork and customer service in all interactions.
  • Responsible for development of policies and procedures within Social Work.
  • Keeps Director updated on issues, problems and progress towards goals.
  • Keeps current in best practices in, Social Work, Case Management, Discharge Planning and Utilization Review.
  • May supervise other case management staff administratively/operationally when needed.
  • Perform other duties as required.
Education/Experience Requirements

REQUIRED:    

  • Master's Degree in Social Work (MSW) from an accredited institution.
  • 3 years of experience in Social Work with 1 year in a lead or supervisory role. 
  • Demonstrated job knowledge, professionalism, critical thinking and leadership skills.
  • Excellent verbal, written and interpersonal skills.
    Proficient with MS Office and EHR (Epic).
  • Ability to handle high-stress situations with professionalism and sensitivity.
  • Ability to travel to other worksites within MVHS.

PREFERRED:

  • Prior supervisory experience.
  • Knowledge of EPIC.
  • Experience in a hospital inpatient or outpatient environment.
Licensure/Certification Requirements

REQUIRED:    

  • Current and valid New York State licensure as a LCSW.

PREFERRED:

  • Current and valid New York State licensure as a LCSW
  • If LMSW, willing to obtain LCSW certification within 6 months.
