Job description

$ 40.4563 to $ 49.7373 per hour

The Adult Day Care Center located @ Waters Place in the Bronx, is seeking a temporary Social Worker to join our team! This position is 30 - 37.5 hours per week, Monday - Friday, 8:30 AM - 3:00 or 4:30PM, including alternating Saturdays 8:30 AM - 3:30 PM.

Job Responsibilities:

  • Work as part of an interdisciplinary team to provide comprehensive MDS and psychosocial assessments for clients in our Dementia Care our Enhanced Memory Impaired program.
  • Coordinate and facilitate care planning and completion of all documentation
  • Facilitate team and family meetings, education on older adult's rights and well-being, needs of the vulnerable elderly population. This involves extensive work with family members and friends.

Job Qualifications:

  • MSW from an accredited School of Social Work.
  • Valid New York State License in Social Work. (LMSW)
  • Five (5) years dementia care experience
  • Bi-lingual (English/Spanish) a plus
  • Certified in Dementia Care a plus
  • Experience in adult day care center preferable.
  • Solid written, communication and organizational skills
