Job description

Patient Service Specialist / Care Coordinator

Are you a compassionate mission-driven person looking to make a meaningful impact on the lives of complex medical patients? As a PSS-Care Coordinator at P3 Health Partners, you'll bring hope and healing to patients with complex medical needs. In this full-time role, you'll play a vital role as a team member, transforming lives and empowering primary care physicians to deliver exceptional care to those who need it most.

As a PSS-Care Coordinator, you will enthusiastically welcome patients and their caretakers as they enter a P3 Health Partner facility developing a sense of trust and confidence in the care they are about to receive. This concierge-like approach levels the patient's positive experience throughout their visit. Additional accountabilities include, but are not limited to, answering phones and scheduling patients. The care coordinator portion of the position entails gathering pertinent data from multiple sources prior to the patient's appointment, allowing the care team to effectively manage the patient at the time of their visit. The care coordinator visit prep extends to all patients in comprehensive care be it an appointment at the facility or at a site external to the facility.

This position will be at P3 Health Partners' Omaha Wellness Center Monday - Friday 8:00 AM - 5:00 PM. This is not a remote position.

Essential Functions:

  • Positively and enthusiastically greet patients and their visits when entering the facility.
  • Provide concierge customer service while a patient is awaiting and following their appointment.
  • Assist in making follow-up visits or scheduling follow-up testing.
  • Work collaboratively with P3 Health Partner teams and their affiliates.
  • Following role-based playbooks, prep a patient's chart by gathering necessary information from multiple sources prior to a patient's visit.
  • Obtain requested data or test results from multiple potential sources when requested by a team member.
  • Update and maintain accurate patient contact information.
  • Serve as a positive and collaborative team member.
  • Uphold high standards of excellence and professionalism.
  • Respond positively to make any necessary adjustments to manager feedback.
  • Confidently use technology to positively impact outcomes.
  • Ensure patient privacy and adherence to all HIPAA regulations and standards for handling PHI.
  • Participate in quality improvement initiatives and operational efficiency.
  • Maintain compliance with required trainings, meeting attendance, timely responses to emails, and other requirements.
  • Role model the P3 Health Partners core values.

Qualifications:

  • High school completion
  • Experience in a service industry role preferred
  • Flexible, self-driven, collaborative and innovative personality

Position Location: Omaha, Nebraska

Pay Range: $17.50 - $22.00 per hour based on experience
