Job Description
Our client is seeking a compassionate and dedicated Case Navigator to provide comprehensive case management services to individuals living with HIV/AIDS. In this role, you will assess client needs, develop individualized service plans, connect clients to housing and community resources, and advocate for their long-term stability and well-being. The ideal candidate is empathetic, organized, and passionate about helping clients navigate complex systems while maintaining accurate documentation and providing trauma-informed care.

Responsibilities:
  • Conduct comprehensive client assessments and develop individualized service plans using the EMPath case management model.
  • Assist clients in securing stable housing, accessing healthcare, and connecting with community resources and entitlement programs.
  • Provide crisis intervention, ongoing case management, and advocacy to support client goals and housing stability.
  • Maintain accurate case files, documentation, and database records while ensuring confidentiality and compliance with program requirements.
  • Collaborate with landlords, healthcare providers, and community partners to coordinate services and improve client outcomes.
Qualifications and Requirements:
  • Associate degree in Psychology, Social Work, Human Services, or a related field required.
  • Minimum of one (1) year of case management or human services experience.
  • Valid Texas Driver's License, reliable transportation, and current vehicle insurance required.
  • Ability to obtain National Standards for the Physical Inspection of Real Estate (NSPIRE) certification within six months of employment.
  • Strong communication, organizational, problem-solving, and interpersonal skills with the ability to work independently and within a team.
