Registered Nurse, Case Management in Grand Junction, Colorado, US. Start date: 08/31/2026. 8-week travel temporary assignment, 36 guaranteed hours per week. Full-Time with weekend coverage requirements. This role operates in an inpatient care setting with an interdisciplinary care team, focusing on developing, implementing, and evaluating the Case Management Plan to meet patients’ health and human service needs. The RN Case Manager provides advocacy, coordination, and education while aligning care with regulatory and payer requirements.

Responsibilities
- Develop, implement, and evaluate case management plans for patient populations, coordinating care across disciplines.

- Maintain current knowledge of discharge planning, utilization processes, and case management best practices through ongoing education.
Requirements
- Current, unrestricted Registered Nurse license in Colorado
- Bachelor’s Degree in Nursing; Master’s degree preferred
- At least three (3) years of clinical experience in a hospital setting and at least five (5) years of case management experience in a hospital setting
- Case management certification preferred or willingness to obtain within two (2) years of employment
- Ability to document and communicate clearly; strong collaboration with physicians, patients, families, and staff
- Willingness to fulfill weekend coverage and participate in the described weekend rotation
